摘要
目的分析磷酸化的mTOR、p70S6K及4EBP1在正常组织、高/低级别上皮内瘤变组织及食管鳞癌组织中的表达水平差异及其与患者临床病理特征的相关性,为早期诊断、治疗食管癌提供依据。方法收集食管鳞状细胞癌组织89例,并选取周围高级别上皮内瘤变组织34例,低级别上皮内瘤变组织17例,及远离肿瘤的正常食管组织59例,制作组织芯片,运用免疫组织化学技术检测p-mTOR、p-p70S6K及p-4EBP1在各组中的表达,并与临床病理参数进行相关性分析。结果 p-mTOR、p-p70S6K和p-4EBP1水平在食管鳞癌发生过程中呈递增趋势。p-mTOR水平与肿瘤病理分级呈正相关,在粘膜层及纤维膜层显著高于肌层;p-p70S6K水平与肿瘤临床分期和浸润深度呈负相关。结论 p-mTOR、p-4EBP1与食管癌发生、进展有关,p-p70S6K是食管癌变的早期事件,联合检测可对早期食管癌的发生具有一定预警作用。
Objective To analyze the difference in the expression of phosphorylated mTOR,p70S6K and 4EBP1 in normal tissues,high/low-grade intraepithelial neoplasia tissues and esophageal squamous cell carcinoma tissues,investigate their correlation with the clinicopathological characteristics of patients,and provide evidence for early diagnosis and treatment of esophageal cancer.Methods 89 cases of esophageal squamous cell carcinoma,34 cases of high-grade intraepithelial neoplasia and 17 cases of low-grade intraepithelial neoplasia around the tumor,and 59 cases of normal esophageal tissues away from the tumor were selected to make a tissue chip.Immunohistochemistry staining was applied to detect the expression of p-mTOR,p-p70S6K and p-4EBP1 protein in different tissue types and further to analyze their correlation with clinicopathological parameters.Results The level of p-mTOR,p-p70S6K and p-4EBP1 increased during the cancerization of esophageal squamous cell carcinoma.The level of p-mTOR was positively correlated with the pathological grade of the tumor,and its expression in the mucous membrane and fibrous membrane was significantly higher than that in the muscle layer.The level of p-p70S6K was negatively correlated with the clinical stage of the tumor and the depth of invasion.Conclusion p-mTOR and p-4EBP1 are related to the occurrence and progression of esophageal cancer,and p-p70S6K is an early event of esophageal cancerization.Joint detection of these three indicators can warn of the occurrence of early esophageal cancer.
