TPF-D对老年2型糖尿病合并重症下呼吸道感染患者炎症免疫水平及血清重组β-细胞营养因子的影响 被引量:2

摘要 目的:探究含缓释淀粉EN制剂(瑞代)(TPF-D)在老年2型糖尿病合并重症下呼吸道感染中的应用效果及其对炎症免疫水平、血清重组β-细胞营养因子的影响。方法:选取我科2019年9月至2021年8月期间129例老年2型糖尿病合并重症下呼吸道感染患者作为研究对象,采用随机数字表法将其分为两组,对照组64例给予百普力乳剂,观察组65例给予TPF-D,两组均使用鼻肠管持续滴注方式,干预后对比两组患者营养状态,炎症免疫水平,糖代谢,不良反应发生率。结果:治疗前,两组营养状态、炎症免疫水平、糖代谢指标比较无统计学意义(P>0.05);治疗后,观察组血清白蛋白(Albumin,Alb)、前白蛋白(Prealbumin,PA)和转铁蛋白(Transferrin,TRF)、免疫球蛋白G(Immunoglobulin G,IgG)和免疫球蛋白A(Immunoglobulin A,IgA)高于对照组,C-反应蛋白(C-reactive protein,CRP)、降钙素原(Procalcitonin,PCT)、重组β-细胞营养因子、胰岛素使用量水平低于对照组,(P<0.05);观察组治疗前后血糖水平比较无统计学意义(P>0.05);两组不良反应比较无统计学意义(P>0.05)。结论:TPF-D不仅可以改善老年2型糖尿病合并重症下呼吸道感染患者的营养状态,同时控制患者血糖,降低炎性反应状态,提高免疫力。 Objective:To explore the effects of EN preparations containing sustained-release starch(Redai)(TPF-D)in elderly type 2 diabetes with severe lower respiratory tract infections and their effects on inflammation and immune levels and serum recombinantβ-cytotrophic factors Influence.Methods:129 elderly patients with type 2 diabetes with severe lower respiratory tract infection in our department from September 2019 to August 2021 were selected as the research subjects,and they were divided into two groups by random number table method.64 cases in the control group were given Baipu For Liemulsion.65 patients in the observation group were given TPF-D.Both groups were given continuous infusion of naso-intestinal tubes.After intervention,the nutritional status,inflammatory and immune levels,glucose metabolism,and incidence of adverse reactions were compared between the two groups.Results:Before treatment,the nutritional status,inflammatory immune level,and glucose metabolism index of the two groups were not statistically significant(P>0.05).After treatment,the observation group serum albumin(albumin,Alb),prealbumin(Prealbumin,PA)and conversion Ferritin(transferrin,TRF),immunoglobulin G(immunoglobulin G,IgG)and immunoglobulin A(immunoglobulin A,IgA)were higher than the control group.C-reactive protein(CRP),calcitonin procalcitonin(PCT),recombinantβ-cytotrophic factor,and insulin consumption were lower than those of the control group(P<0.05).The blood glucose levels of the observation group before and after treatment were not statistically significant(P>0.05).The adverse reactions between the two groups The comparison was not statistically significant(P>0.05).Conclusion:TPF-D can not only improve the nutritional status of elderly patients with type 2 diabetes complicated with severe lower respiratory tract infection,but also control the blood sugar of the patients,reduce the inflammatory response state,and improve immunity.
