中医清创术联合除癖消溢散治疗浆细胞性乳腺炎临床研究

Clinical Study on Debridement of TCM Combined with Chupi Xiaoyi Powder in the Treatment of Plasma Cell Mastitis

目的:观察中医清创术联合除癖消溢散治疗浆细胞性乳腺炎的临床疗效。方法:将86例浆细胞性乳腺炎患者按照随机数字表法分为对照组和治疗组,每组各43例。入院后两组均患者接受中医清创术治疗,对照组在术后给予常规西药对症治疗,观察组在对照组的基础上给予除癖消溢散治疗。比较两组患者的临床疗效及治疗前后炎性因子水平[肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、白细胞介素(interleukin,IL)-6、IL-8]、免疫功能指标[血清补体C3、免疫球蛋白(immunoglobulin G,IgG)、免疫球蛋白A(immunoglobulin A,IgA)]、中医证候积分变化情况。结果:两组患者治疗后TNF-α、IL-6、IL-8、补体C3、IgG、IgA水平低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后中医证候积分低于本组治疗前,且治疗后观察组低于对照组,差异具有统计学意义(P<0.05)。观察组有效率为93.02%,明显高于对照组的74.42%,两组患者有效率比较,差异具有统计学意义(P<0.05)。结论:中医清创术联合除癖消溢散治疗浆细胞性乳腺炎,能够减轻机体炎症反应,改善免疫功能。

Objective:To observe the clinical curative effect of debridement of TCM combined with Chupi Xiaoyi Powder(除癖消溢散,CPXYP)on plasma cell mastitis.Methods:A total of 86 patients with plasma cell mastitis were randomly divided into the control group and the observation group,with 43 cases in each group.After admission,both groups were treated with debridement of TCM.The control group was treated with routine Western medicine symptomatic treatment,while the observation group was treated with CPXYP on the basis of the control group.The clinical efficacy,the changes of the levels of tumor necrosis factor-α(TNF-α),interleukin(IL)-6,IL-8],immune function indexes[including serum complement C3,immunoglobulin G(IgG),immunoglobulin A(IGA)]before and after treatment were compared between the two groups.Results:After treatment,the levels of TNF-α,IL-6,IL-8,complement C3,IgG and IgA were lower than those of the same group before treatment,and the above indexes in the observation group were lower than those in the control group,and all the differences were statistically significant(P<0.05).The TCM syndrome score of the two groups after treatment was lower than that of the same group before treatment,and that index of the observation group after treatment was lower than that of the control group,and the difference was statistically significant(P<0.05).The effective rate of the observation group was 93.02%,which was significantly higher than 74.42%of the control group.There was significant difference between the two groups(P<0.05).Conclusion:Debridement of TCM combined with CPXYP in the treatment of plasma cell mastitis can reduce the inflammatory response and improve immune function.
