清心镇肝汤治疗肝郁化火型失眠临床研究

Clinical Study on Heart-Clearing and Liver-Settling Decoction in the Treatment of Insomnia with Syndrome of Liver Depression Transforming into Fire

目的:观察清心镇肝汤治疗肝郁化火型失眠的临床疗效。方法:将112例肝郁化火型失眠患者按照随机数字表法分为对照组和研究组,每组各56例。对照组给予佐匹克隆治疗,研究组给予清心镇肝汤治疗。比较两组患者的临床疗效、不良反应发生率及治疗前后睡眠质量评分变化情况。检测两组患者治疗前后血清核转录因子-κB(nuclear transcription factor-κB,NF-κB)、脑源性神经营养因子(brain derived neurotrophic factor,BDNF)和5-羟色胺(5-hydroxy tryptamin,5-HT)水平。结果:两组患者治疗后睡眠质量评分低于本组治疗前,且治疗后研究组低于对照组,差异具有统计学意义(P<0.05)。两组患者治疗后NF-κB水平低于本组治疗前,BDNF、5-HT高于本组治疗前,且治疗后研究组NF-κB水平低于对照组,BDNF、5-HT高于对照组,差异具有统计学意义(P<0.05)。对照组有效率为82.1%,研究组有效率为96.4%,两组患者有效率比较,差异具有统计学意义(P<0.05)。对照组不良反应发生率为21.4%,研究组不良反应发生率为3.6%,两组患者不良反应发生率比较,差异具有统计学意义(P<0.05)。结论:清心镇肝汤治疗肝郁化火型失眠,能有效改善患者睡眠质量,调节炎性因子水平。

Objective:To observe the clinical curative effect of Heart-Clearing and Liver-Settling Decoction on insomnia with syndrome of liver depression transforming into fire.Methods:A total of 112 patients with insomnia with syndrome of liver depression transforming into fire were randomly divided into the control group and the study group,with 56 cases in each group.The control group was treated with conventional Western medicine,while the study group was treated with Heart-Clearing and Liver-Settling Decoction.The clinical efficacy,the incidence of adverse reactions and the changes of sleep quality score before and after treatment were compared between the two groups.The levels of serum nuclear transcription factor-κB(NF-κB),brain-derived neurotrophic factor(BDNF)and 5-hydroxytryptamine(5-HT)before and after treatment were detected.Results:The sleep quality score of the two groups after treatment was lower than that of the same group before treatment,and the sleep quality score of the study group after treatment was lower than that of the control group,and the difference was statistically significant(P<0.05).After treatment,the NF-κB level of the two groups was lower than that of the same group before treatment,but BDNF and 5-HT were higher than that of the same group before treatment;after treatment,the NF-κB level of the study group was lower than that of the control group,while the levels of BDNF and 5-HT were higher than those of the control group,and all the differences were statistically significant(P<0.05).The effective rate was 82.1%in the control group and 96.4%in the study group.There was significant difference between the two groups(P<0.05).The incidence of adverse reactions was 21.4%in the control group and 3.6%in the study group.There was significant difference between the two groups(P<0.05).Conclusion:Heart-Clearing and Liver-Settling Decoction can effectively improve patients′sleep quality and regulate the level of inflammatory factors in the treatment of insomnia with syndrome of liver depression transforming into fire.
