祛瘀散寒中药联合坦索罗辛治疗慢性骨盆疼痛综合征临床观察

Clinical Observation of Removing Blood Stasis for Dispelling Cold Chinese Medicine Combined with Tamsulosin in the Treatment of Chronic Pelvic Pain Syndrome

目的分析祛瘀散寒中药联合坦索罗辛对慢性骨盆疼痛综合征患者炎症反应指标及中医证候的影响。方法选取慢性骨盆疼痛综合征患者80例,随机分为联合组(40例)、对照组(40例)。对照组接受坦索罗辛治疗,联合组在对照组基础上采用祛瘀散寒中药治疗,2组均治疗1个月。比较2组治疗前后NIH-CPSI评分、炎症反应、中医证候积分及治疗期间安全性。结果治疗后与治疗前比较,2组前列腺液中IL-2水平均升高,联合组高于对照组(P<0.05)。治疗后与治疗前比较,2组前列腺液中IL-1β、TNF-α、IFN-γ水平,疼痛不适、排尿症状、生活质量评分,中医证候积分(尿频尿急、尿后滴沥、排尿刺痛)降低,且联合组低于对照组(P<0.05)。治疗期间,联合组与对照组不良反应发生率比较,差异无统计学意义(1.127%vs0.288%,P>0.05)。结论祛瘀散寒中药联合坦索罗辛可抑制慢性骨盆疼痛综合征患者炎症反应,改善中医证候及临床症状,安全性良好。

Objective To analyze the effect of removing blood stasis for dispelling cold Chinese medicine combined with tamsulosin on inflammatory response indexes and traditional Chinese medicine syndromes in patients with chronic pelvic pain syndrome.Methods 80 patients with chronic pelvic pain syndrome were randomly divided into the combined group(n=40)and the control group(n=40).The control group was treated with tamsulosin,and the combined group was treated with removing blood stasis for dispelling cold Chinese medicine on the basis of the control group.Both groups were treated for 1 month.The NIH-CPSI score,inflammatory reaction,traditional Chinese medicine syndrome score and safety in the two groups were compared.Results After treatment,the levels of IL-2 level in prostatic fluid in the two groups increased,and those in the combined group were higher than the control group(P<0.05).After treatment,compared with before treatment,the levels of IL-1β,TNF-α,IFN-γin prostatic fluid,the scores of pain and discomfort,urination symptoms,quality of life and traditional Chinese medicine syndrome scores(frequent urination,urgency urination,drip after urination,micturition tingling)in the two groups decreased,and those in the combined group were lower than those in the control group(P<0.05).During the treatment,there was no significant difference in the incidence of adverse reactions between the combined group and the control group(1.127%vs 0.288%,P>0.05).Conclusion Removing blood stasis for dispelling cold Chinese medicine combined with tamsulosin could inhibit the inflammatory reaction of patients with chronic pelvic pain syndrome,improve traditional Chinese medicine syndrome and clinical symptoms,and has good safety.
