头皮帽状腱膜层下毫针针刺松解治疗偏头痛临床观察

Analysis on the Clinical Effect of Filiform Needle Acupuncture Release under Scalp Cap-shaped Aponeurosis in the Treatment of Migraine

目的分析头皮帽状腱膜层下毫针针刺松解治疗偏头痛的临床效果。方法选取2016年10月—2018年11月收治的偏头痛患者80例,按照就诊的先后顺序分为治疗组和对照组。治疗组采用头皮帽状腱膜层下毫针针刺松解治疗,对照组采用传统辨证取穴法合补泻结合治疗,分析2组患者治疗后的临床疗效。结果治疗组总有效率为92.50%,对照组为75.00%,治疗组患者总有效率高于对照组(P<0.05)。2组治疗前头痛发作频率和每次持续时间组间比较,差异不具有统计学意义(P>0.05)。2组治疗后头痛发作频率较治疗前下降,每次持续时间较治疗前缩短,治疗组治疗后头痛发作频率低于对照组,每次持续时间短于对照组(P<0.05)。2组治疗前血流动力学指标水平组间比较,差异不具有统计学意义(P>0.05)。2组治疗后ACA、MCA、PCA较治疗前下降,治疗组治疗后ACA、MCA、PCA水平低于对照组(P<0.05)。2组治疗前5-HT、ET-1、CGRP水平组间比较,差异不具有统计学意义(P>0.05)。2组治疗后5-HT较治疗前升高,ET-1、CGRP较治疗前下降,治疗组治疗后5-HT水平高于对照组,ET-1、CGRP水平低于对照组(P<0.05)。结论头皮帽状腱膜层下毫针针刺松解治疗偏头痛疗效显著,可调节患者5-HT、ET-1、CGRP的表达水平,改善血流动力学,降低头痛发作频率。

Objective To analyze the clinical effect of filiform needle acupuncture release under scalp cap-shaped aponeurosis in the treatment of migraine.Methods From October 2016 to November 2018,80 patients with migraine were divided into treatment group and control group.The treatment group was treated with scalp cap-shaped aponeurosis,while the control group was treated with traditional syndrome differentiation acupoint selection combined with reinforcing and reducing.The clinical effects of the two groups were analyzed.Results The total effective rate was 92.50%in the treatment group and 75.00%in the control group,and the total effective rate in the treatment group was higher than that in the control group(P<0.05).There was no significant difference in the frequency and duration of headache between the two groups before treatment(P>0.05).After treatment,the frequency of headache attack in the two groups was lower than before,the frequency of headache attack after treatment in the treatment group was lower than that in the control group,and the duration of each time was shorter than that in the control group(P<0.05).There was no significant difference in the hemodynamic indexes between the two groups before treatment(P>0.05).After treatment,the ACA,MCA and PCA of the two groups decreased compared with that before treatment.The level of ACA,MCA and PCA in the treatment group after treatment was lower than that in the control group(P<0.05).There was no significant difference in the level of 5-HT,ET-1 and CGRP between the two groups before treatment(P>0.05).After treatment,The 5-HT in group 2 was higher than that before treatment,ET-1 and CGRP decreased,the level of 5-HT in the treatment group was higher than that in the control group,and the level of ET-1 and CGRP was lower than that in the control group(P<0.05).Conclusion Acupuncture under scalp cap-shaped aponeurosis is effective in the treatment of migraine.It could regulate the expression level of 5 HT,ET-1,CGRP,improve hemodynamics and decrease the frequency of headache attack in patients with migraine.
