肛肠手术50例术后直肠出血临床分析

Clinical Analysis of 50 Cases of Rectal Bleeding after Anorectal Operation

目的分析肛肠手术患者术后的直肠出血情况。方法选取医院2018年10月至2019年10月收治的50例肛肠手术患者作为研究对象,对其病历资料进行回顾性分析,记录术后直肠出血发生情况,包括术后出血时间、出血量、治疗效果、住院时间等,统计直肠出血的发生率;分析直肠出血的原因,探讨其治疗方案,并提出相应的预防措施。结果50例患者中,术后发生直肠出血4例,发生率为8.0%,包括肛瘘切开术1例,混合痔外剥内扎术1例,直肠脓肿切开引流术1例,直肠放射切口瘢痕松解术1例。4例患者均于术后3~10 d发生直肠出血,平均出血时间为(5.20±0.60)d;出血量为44~241 mL,平均出血量为(133.90±74.50)mL,经及时处理与治疗,均成功止血,无死亡病例;所有患者均治愈出院,住院时间为13~27 d,平均住院时间为(18.40±3.80)d。直肠出血原因中,创面损伤2例,占比50.00%,术后活动动作过大1例,占比25.00%,未及时处理血管丰富部位的切口1例,占比25.00%。结论肛肠手术后直肠出血的发生率较高,其影响因素多,应做好预防措施,一经发现必须及时处理与治疗,以确保手术效果,改善患者预后。

Objective To analyze the rectal bleeding after anorectal operation.Methods A total of 50 cases of anorectal operation patients admitted to our hospital from October 2018 to October 2019 were selected as the research objects,and their medical records were analyzed retrospectively.The occurrence of postoperative rectal bleeding was recorded,including postoperative bleeding time,bleeding volume,treatment effect,hospital stay,etc.,and the incidence of rectal bleeding was calculated.The causes of rectal bleeding were analyzed,and the treatment plan was discussed,and the treatment plan was put forward Corresponding preventive measures.Results Among the 50 patients,4 had rectal bleeding,the incidence was 8.00%,including 1 case of anal fistula incision,1 case of mixed hemorrhoids exfoliation and internal ligation,1 case of rectal abscess incision and drainage,1 case of rectal radiation incision scar release.Four patients all had rectal bleeding 3-10 days after operation,the average bleeding time was(5.20±0.60)day;the bleeding volume was 44-241 mL,the average bleeding volume was(133.90±74.50)mL,after timely treatment and treatment,all patients were successful in hemostasis,no death cases;all patients were cured and discharged,the hospitalization time was 13-27 days,the average hospitalization time was(18.40±3.80)day.Among the causes of rectal bleeding,wound injury was found in 2 cases,accounting for 50.0%,excessive postoperative movement in 1 case,accounting for 25.00%,and incision with abundant blood vessels was not treated in time in 1 case,accounting for 25.00%.Conclusion The incidence of rectal bleeding after anorectal operation is high,and there are many factors affecting it.Preventive measures should be taken.Once found,it must be treated and treated in time to ensure the effect of operation and improve the prognosis of patients.
