中药浸浴熏洗治疗四肢Ⅱ度烧伤创面临床疗效观察 被引量:3

Clinical Curative Observation of Applying Traditional Chinese Medicine Immersion Bath Fumigation in the Treatment of Second-degree Limb Burn Wounds

摘要 目的:观察冬菊洗剂浸浴熏洗治疗四肢Ⅱ度烧伤创面临床疗效。方法:回顾性研究2019年12月~2020年12月在我院进行治疗的116例四肢Ⅱ度烧伤患者相关临床资料,根据患者治疗方式分为熏洗组60例和常规组56例,常规组患者创面处理后采用生理盐水浸泡,常规碘伏消毒,涂抹磺胺嘧啶银乳膏,内层为碘伏油纱,外层为多层无菌纱布包扎,熏洗组患者创面处理后采用冬菊洗剂浸泡,浸泡后在进行冬菊洗剂熏洗,常规碘伏消毒后涂抹磺胺嘧啶银乳膏,使用与常规组相同的方法包扎,两组患者隔天换药一次直至愈合,对比两组患者临床恢复相关指标,治疗14天后炎性因子水平、生长因子水平及蛋白质水平变化。结果:熏洗组患者第14天创面愈合率低于常规组,创面最终愈合时间及换药次数少于常规组(P<0.05);两组患者治疗后C反应蛋白(CRP)、肿瘤坏死因子(TNF-α)及白细胞介素-6(IL-6)水平均下降,且治疗后熏洗组患者上述炎性因子水平均低于常规组(P<0.05);两组患者治疗后血管内皮生长因子(VEGF)、碱性成纤维细胞生长因子(bFGF)及转化生长因子-β1(TGF-β1)水平均上升,且治疗后熏洗组患者上述生长因子水平均高于常规组(P<0.05);两组患者治疗后前白蛋白、白蛋白及转铁蛋白水平均高于治疗前,且治疗后熏洗组患者前白蛋白、白蛋白及转铁蛋白水平均高于常规组(P<0.05)。结论:冬菊洗剂浸泡熏洗治疗可加快四肢Ⅱ度烧伤患者恢复速度,减少换药次数,降低患者炎性因子水平,提高患者生长因子水平含量,提高患者蛋白质代谢能力。 Objective: To explore the clinical efficacy of applying immersion bath fumigation with Dongju Lotion in treating second-degree limbs burn wounds.Methods: A retrospective study was performed on the clinical data of 116 patients with second-degree limb burns who were treated in the hospital between December 2019 and December 2020.According to the treatment methods of patients, they were divided into a fumigation group(60 cases) and a conventional group(56 cases).Patients in the conventional group after wound treatment were soaked in normal saline, routinely disinfected with iodophor, and applied with Sulfadiazine Silver Cream(inner layer was iodophor gauze and outer layer was wrapped with multi-layer sterile gauze, and patients in the fumigation group after wound treatment were soaked with Dongju Lotion, and then fumigated with Dongju Lotion after soaking, and were applied with Sulfadiazine Silver Cream after conventional iodophor disinfection(with the same method of bandaging as the conventional group),and the two groups of patients changed dressing every other day until they healed.The clinical recovery-related indicators and changes in levels of inflammatory factors, growth factors and proteins after 14 days of treatment were compared between the two groups.Results: The wound healing rate of the fumigation group on the 14;day was lower than that of the conventional group, and the final wound healing time and the frequency of dressing changes were shorter or less than those of the conventional group(P<0.05).The levels of C-reactive protein(CRP),tumor necrosis factor(TNF-α) and interleukin-6(IL-6) were decreased in the two groups after treatment, and the levels of the above-mentioned inflammatory factors in the fumigation group after treatment were lower than those in the conventional group(P<0.05).The levels of vascular endothelial growth factor(VEGF),basic fibroblast growth factor(bFGF) and transforming growth factor-β1(TGF-β1) of the two groups of patients were increased after treatment, and the levels of above-mentioned growth factors of patients after treatment were higher in the fumigation group than those in the conventional group(P<0.05).The levels of prealbumin, albumin and transferrin of the two groups of patients after treatment were higher than those before treatment, and the levels of prealbumin, albumin and transferrin of patients in the fumigation group were higher than those in the conventional group after treatment(P<0.05).Conclusion: Immersion bath fumigation with Dongju Lotion can speed up the recovery of patients with second-degree limb burns, reduce the frequency of dressing changes, lower the levels of inflammatory factors, increase the levels of growth factors, and improve the protein metabolism of patients.
