摘要 目的探究液基薄层细胞学检查(TCT)联合人乳头状瘤病毒(HPV)、P16蛋白免疫组化染色检测对宫颈癌的筛查价值。方法2018年2月至2019年3月,采用TCT和HPV检测至少有一项异常的80例患者作为研究对象,进行P16蛋白免疫组化染色,并进行阴道镜活检检验作为标准,评价TCT、HPV以及P16联合检测对诊断宫颈癌早期病变的意义。结果宫颈病变程度越高,TCT鳞状上皮细胞异型越明显,HPV检测的阳性率越高,P16蛋白阳性率越高。且≤CINⅠ组的P16蛋白表达的阳性率明显低于≥CINⅡ组(P<0.05)。HPV、TCT、P16联合检测的灵敏性、特异性、准确性均高于单独HPV检测、TCT检测、P16检测。结论通过对宫颈癌早期病变患者,采用液基薄层细胞学检查(TCT)及其联合人乳头状瘤病毒(HPV)、P16蛋白免疫组化染色检测,能够提高检测灵敏度、特异性以及准确度。 Objective To investigate the screening value of liquid-based thin-prep cytology test(TCT)combined with human papillomavirus(HPV)and P16 protein immunohistochemical staining for cervical cancer.Methods From February 2018 to March 2019,80 patients with at least one abnormality were examined by TCT and HPV.P16 immunohistochemical staining was performed,and colposcopy biopsy was performed as a standard to evaluate TCT,HPV,and the significance of combined detection of P16 in the diagnosis of early cervical cancer.Results The higher the degree of cervical lesions,the more obvious the abnormality of TCT squamous epithelial cells.The higher the positive rate of HPV detection,the higher the positive rate of P16 protein.The positive rate of P16 protein expression in≤CINⅠgroup was significantly lower than that in≥CINⅡgroup(P<0.05).The sensitivity,specificity and accuracy of HPV,TCT and P16 combined detection were higher than HPV detection,TCT detection and P16 detection.Conclusion By using liquid-based thin-prep cytology test(TCT)and its combined human papillomavirus(HPV)and P16 protein immunohistochemical staining for early cervical cancer patients,detection sensitivity,specificity and accuracy can be improved.
