腹腔镜下子宫肌瘤剔除术与传统开腹剔除术治疗子宫肌瘤的效果及对疼痛评分、炎性因子水平的影响

Study on the effect of Laparoscopic myomectomy and Traditional laparoscopic myomectomy in the Treatment of uterine fibroids and the influence of pain score and Inflammatory factors

目的探讨腹腔镜下子宫肌瘤剔除术与传统开腹剔除术治疗子宫肌瘤的效果及对患者疼痛评分、炎性因子水平的影响。方法选取74例子宫肌瘤患者,根据手术方法不同分为对照组与试验组,各37例。对照组实施传统开腹剔除术治疗,试验组实施腹腔镜下子宫肌瘤剔除术。对比两组手术相关指标、术后并发症发生情况及炎性因子水平。结果试验组术中失血量明显少于对照组,术后肠胃功能恢复时间、住院时间均明显短于对照组,术后疼痛评分低于对照组,差异有统计学意义(P<0.05)。试验组术后C反应蛋白(CRP)、白细胞介素-6(IL-6)水平均明显低于对照组,差异有统计学意义(P<0.05)。试验组术后并发症发生率为2.7%,明显低于对照组的16.2%,差异有统计学意义(P<0.05)。结论子宫肌瘤应用传统开腹剔除术与腹腔镜下子宫肌瘤剔除术治疗均可实现治疗目的,但腹腔镜下子宫肌瘤剔除术创伤较小,可降低炎性因子水平,且减轻疼痛感,术后并发症少,具有临床应用价值。

Objective To investigate the effect of laparoscopic myomectomy and traditional laparotomy in the treatment ofhysteromyoma and the effect on pain score and inflammatory factors.Methods 74 patients with uterine fibroids were selected and divided into control group and experimental group according to different surgical methods,37 cases in each group.The control group was treated with traditional laparotomy,while the experimental group was treated with laparoscopic myomectomy.Operative indicators,postoperative complications and inflammatory factors were compared between the two groups.Results The intraoperative blood loss of experimental group was significantly less than the control group,postoperative gastrointestinal function recovery time,hospitalization time were significantly shorter than the control group,postoperative pain score was lower than the control group,the difference was statistically significant(P<0.05).The levels of C-reactive protein(CRP)and interleukin-6(IL-6)in experimental group were significantly lower than those in control group,the difiFerences were statistically significant(P<0.05).The incidence of postoperative complications in the experimental group was 2.7%,which was significantly lower than that in the control group(16.2%),with statistical significance(P<0.05).Conclusion Both traditional laparotomy and laparoscopic myomectomy can achieve the therapeutic purpose of myomectomy,but laparoscopic myomectomy has less trauma,can reduce the level of inflanunatory factors,relieve pain,and less postoperative complications,and has specific clinical application value.
