宫颈扩张球囊在瘢痕子宫足月妊娠阴道试产中的应用效果评价

Evaluation of the effect of cervical dilatation balloon in vaginal delivery of full-term pregnancy with cicatricial uterus

目的分析宫颈扩张球囊在瘢痕子宫足月妊娠阴道试产中的应用效果。方法选取2020年1至10月于本院分娩的96例存在瘢痕子宫的产妇,按照随机数字表法分为两组,各48例。对照组采用宫缩素,研究组采用宫颈扩张球囊。比较妊娠结局、母婴不良结局、新生儿Apgar评分、促宫颈成熟度及引产效果、引产时间及产程、产后24h出血量。结果研究组阴道分娩率、促宫颈成熟率、引产成功率、引产至临产成功时间、第一产程时间高于对照组(P<0.05);两组剖宫产、机械助产率、母婴不良结局发生率、新生儿Apgar评分、产后24h内出血量、引产成功80%率、成功50%率及失败率比较,差异无统计学意义(P>0.05)。结论宫颈扩张球囊在瘢痕子宫足月妊娠阴道试产中具有良好的应用效果,有利于促进宫颈成熟,缩短引产时间,提高引产成功率及阴道自然分娩率,并对母婴不良结局无太大影响。

Objective:To analyze the effect of cervical dilatation balloon in vaginal delivery of full-term pregnancy with cicatricial uterus.Methods:We selected 96 women who delivered in our hospital from January 2020 to October 2020,and divided them into two groups according to random number table,48 cases in each group.The control group used Oxytocin,the study group used cervical dilatation balloon.The pregnancy outcome,maternal and infant adverse outcome,Apgar score,cervical maturity and induction effect,induction time and duration of labor,24h postpartum hemorrhage were compared.Results:The vaginal delivery rate,cervical ripening rate,successful induction of labor,the time from induction to parturition and the first stage of Labor in the study group were higher than those in the control group(P<0.05)There was no significant difference between the two groups in the rate of Cesarean section,the rate of mechanical delivery,the rate of adverse outcome of mother and infant,Apgar score of newborn,the amount of bleeding within 24 hours after delivery,the success rate of induced labor 80%,50%and the failure rate(P>0.05).Conclusion:Cervical dilatation balloon has a good application effect in the vaginal trial of full-term pregnancy with scarred uterus,which is beneficial to promote cervical maturity,shorten the induction time,increase the success rate of induction and the rate of natural vaginal pregnancy,and has no significant impact on the adverse outcomes of mothers and babies.
