经皮经肝胆管穿刺置管引流术联合介入手术治疗老年恶性梗阻性黄疸的效果分析

Effect analysis of percutaneous perbiliary catheter drainage combined with interventional operation in the treatment of malignant obstructive jaundice in the aged

目的:研究用经皮经肝胆管穿刺置管引流术联合介入手术治疗老年恶性梗阻性黄疸的效果。方法:将2018年7月至2020年9月期间长江航运总医院收治的98例老年恶性梗阻性黄疸患者纳入本研究。按照随机数表法将其分为引流术组和联合治疗组。用经皮经肝胆管穿刺置管引流术对引流术组患者进行治疗,用经皮经肝胆管穿刺置管引流术联合介入手术对联合治疗组患者进行治疗,然后比较两组患者的疗效及各项临床指标。结果:治疗后,联合治疗组患者血清总胆红素和直接胆红素的水平均低于引流术组患者,P<0.05。联合治疗组患者治疗的总有效率高于引流术组患者,其术后住院的时间短于引流术组患者,P<0.05。两组患者术后并发症的发生率相比,P>0.05。结论:对老年恶性梗阻性黄疸患者进行经皮经肝胆管穿刺置管引流术联合介入手术能显著改善其血清胆红素的水平,缓解其病情,且其术后的并发症较少,住院的时间较短。

Objective:To study the effect of percutaneous transhepatobiliary catheter drainage combined with interventional surgery on malignant obstructive jaundice in the elderly.Methods:A total of 98 elderly patients with malignant obstructive jaundice admitted to Yangtze River General Shipping Hospital from July 2018 to September 2020 were enrolled in this study.They were divided into drainage group and combined treatment group according to random number table method.Patients in the drainage group were treated with percutaneous transhepatobiliary catheterization and drainage,and patients in the combined treatment group were treated with percutaneous transhepatobiliary catheterization and drainage combined with interventional surgery,and then the efficacy and clinical indicators of the two groups were compared.Results:After treatment,the levels of serum total bilirubin and direct bilirubin in combined treatment group were lower than those in drainage group,P<0.05.The total effective rate in the combined treatment group was higher than that in the drainage group,and the postoperative hospitalization time was shorter than that in the drainage group,P<0.05.The incidence of postoperative complications was compared between the two groups,P>0.05.Conclusion:Percutaneous transhepatobiliary catheterization and drainage combined with interventional surgery can significantly improve the level of serum bilirubin in elderly patients with malignant obstructive jaundice,and relieve the disease,and the postoperative complications are less,the hospitalization time is shorter.
