经脐腹腔镜手术治疗新生儿幽门狭窄的疗效探析

Effect of transumbilical laparoscopic surgery on neonatal pyloric stenosis

目的:探讨用经脐腹腔镜手术治疗新生儿幽门狭窄的临床疗效。方法:将无锡市儿童医院2018年3月至2021年3月期间收治的90例新生儿幽门狭窄患儿作为研究对象。根据治疗术式的不同将其分为对照组和试验组。对对照组45例患儿实施常规的开腹手术,对试验组45例患儿实施经脐腹腔镜手术,然后对比两组患儿的治疗效果。结果:1)试验组患儿术后并发症的发生率低于对照组患儿,差异有统计学意义(P<0.05)。2)试验组患儿的手术时间、术后排气时间、住院时间均短于对照组患儿,其术中的出血量少于对照组患儿,差异有统计学意义(P<0.05)。3)治疗后,试验组患儿家长7项广泛性焦虑障碍量表(GAD-7)的评分、9项病人健康问卷抑郁量表(PHQ-9)的评分均低于对照组患儿家长,差异有统计学意义(P<0.05)。结论:用经脐腹腔镜手术治疗新生儿幽门狭窄具有手术时间短、创伤小、患儿术后恢复快、安全性高等优点,可有效地减轻患儿家长的心理负担,缓解其负面情绪。

Objective:To investigate the clinical efficacy of transumbilical laparoscopic surgery for neonatal pyloric stenosis.Methods:A total of 90 children with neonatal pyloric stenosis admitted to Wuxi Children’s Hospital from March 2018 to March 2021 were selected as the study subjects.According to the different surgical methods,they were divided into control group and experimental group.Conventional open surgery was performed in 45 cases in the control group,and transumbilical laparoscopic surgery was performed in 45 cases in the experimental group,and then the therapeutic effects of the two groups were compared.Results:1)The incidence of postoperative complications in the experimental group was lower than that in the control group,the difference was statistically significant(P<0.05).2)The operative time,postoperative exhaust time and hospital stay of the experimental group were shorter than those of the control group,and the amount of intraoperative blood loss was less than that of the control group,with statistical significance(P<0.05).3)After treatment,the scores of 7 items of generalized Anxiety Disorder Scale(GAD-7)and 9 items of patient health Questionnaire depression Scale(PHQ-9)of parents in the experimental group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion:Transumbilical laparoscopic surgery for neonatal pyloric stenosis has the advantages of short operation time,little trauma,quick postoperative recovery and high safety,which can effectively reduce the psychological burden of parents and relieve their negative emotions.
