西宁地区心肺功能正常者动脉及中心静脉血气的变化

摘要 目的观察西宁地区心肺功能正常者动脉、中心静脉血气指标的变化,寻求中心静脉血氧饱和度(ScvO_(2))、静脉动脉二氧化碳差值(Pcv-aCO_(2))及静脉动脉二氧化碳分压差与动脉静脉氧含量差比值(Pcv-aCO_(2)/Ca-cvO_(2))在西宁的参考值。方法收集2019年4月1日至2020年12月31日在青海省人民医院住院的志愿者64例。收集志愿者未吸氧状态下的动脉及中心静脉血气分析指标,比较不同地区/海拔动脉、静脉血气分析指标情况。结果西宁动脉与中心静脉血气分析中pH、二氧化碳分压(PCO_(2))、氧分压(PO_(2))、Ca^(2+)、乳酸(Lac)、HCO3-、碱剩余(BE)和血氧饱和度(SO_(2))的差异均有统计学意义(P均<0.05)。昆明动脉血气分析中pH、PCO_(2)与西宁比较差异均有统计学意义(P均<0.05)。西藏动脉血气分析中PCO_(2)、PO_(2)、BE和SO_(2)与西宁比较差异均有统计学意义(P均<0.05)。西宁与洛杉矶动脉血气分析中PCO_(2)、PO_(2)和SO_(2)的差异均有统计学意义(P均<0.05)。西宁与洛杉矶静脉血气分析中PCO_(2)、PO_(2)和SO_(2)的差异均有统计学意义(P均<0.05)。西宁Pcv-aCO_(2)为(9±3)mmHg,与低海拔指标(6 mmHg)比较差异有统计学意义(P<0.05)。西宁Pcv-aCO_(2)/CacvO_(2)为1.864±0.792。结论中度海拔人群的动脉血气分析及中心静脉血气指标与低海拔地区人群不完全一致,尤其是ScvO_(2)指标在中度海拔人群中明显下降。 Objective To observe the arterial and central venous blood gas changes in patients with normal cardiopulmonary function in Xining area to measure the reference values of central venous oxygen saturation(ScvO_(2)),central venous minus arterial carbon dioxide pressure(Pcv-aCO_(2)),and Pcv-aCO_(2)-to-arterial minus central venous oxygen content ratio(Pcv-aCO_(2)/Ca-cvO_(2))in Xining area.Methods The clinical data of 64 volunteers who were hospitalized at Qinghai Provincial People's Hospital from April 1,2019 to December 31,2020 were collected.And the indexes of arterial and central venous blood gas analysis were also collected from volunteers without oxygen inhalation.The results of arterial and venous blood gas analysis among different regions/altitudes were compared.Results The pH,partial pressure of carbon dioxide(PCO_(2)),partial pressure of oxygen(PO_(2)),Ca^(2+),lactic acid(Lac),HCO3−,base excess(BE),and saturation of peripheral oxygen(SO_(2))differed significantly between venous blood and arterial blood of patients from Xining(P<0.05 for all).The pH and PCO_(2) in arterial blood of patients from Xining were significantly different from those in Kunming(P<0.05 for all).The PCO_(2),PO_(2),BE,and SO_(2) in arterial blood of patients from Xining were all significantly different from those in Tibet(P<0.05 for all).The PCO_(2),PO_(2),and SO_(2) in arterial blood of patients from Xining were significantly different from those in Los Angeles(P<0.05 for all).The PCO_(2),PO_(2),and SO_(2) in venous blood of patients from Xining were significantly different from those in Los Angeles(P<0.05 for all).The Pcv-aCO_(2) in Xining was(9±3)mmHg,which was significantly different from that at low altitude(6 mmHg;P<0.05).The Pcv-aCO_(2)/Ca-cvO_(2) in Xining was 1.864±0.792.Conclusion The arterial and central venous blood gas parameters in the population from medium altitude regions are not completely consistent with those from low altitude regions.ScvO_(2) significantly decreases in the population from medium altitude regions.
