上海市青浦区肺结核密切接触者筛查模式及结果分析

Screening Patterns of Close Contacts of Tuberculosis Patients and Analysis of Screening Results in Qingpu District of Shanghai

目的分析上海市青浦区活动性肺结核患者(指示病例)的密切接触(密接)者筛查情况,探讨肺结核患者密接者的有效筛查模式。方法对上海市青浦区自2016年6月—2020年10月在上海市结核病报告系统中登记的652例户籍指示病例及其1169例密接者的相关信息进行汇总分析,并对密接者的筛查模式及发病情况进行分析研究。结果社区卫生服务中心按照流程对青浦区户籍活动性肺结核患者的密接者进行症状筛查,筛查人数为指示病例的1.79倍;11例密接者被诊断为肺结核患者(0.94%),发病率为941/10^(5)。不同年龄组别、与指示病例不同关系组别和不同发现方式组别的密接者,发病情况差异有统计学意义。结论基层医务人员按照规范的筛查流程对所有痰菌结果指示病例的密接者进行主动筛查,并重点关注18~64岁的家庭成员,是一种提高肺结核患者发现率的有效筛查模式。

Objective To analyze the screening status of close contacts of active pulmonary tuberculosis patients(indicator cases)in Qingpu District of Shanghai,and to explore the effective screening method of close contact of pulmonary tuberculosis patients.Methods From June 2016 to October 2020 in Qingpu District,652 household registration index cases and 1169 cases of close contact were collected and analyzed,and the screening methods and incidence of close contact were analyzed.Results According to the process,the community health service center screened the symptoms of the registered active tuberculosis patients in Qingpu District,and the number of screened cases was 1.79 times of the indicator cases;11 cases(0.94%)were diagnosed as tuberculosis,and the incidence was 941/105.There were significant differences in the incidence of close contact between different age groups,different relationship groups with indicator cases and different discovery methods.Conclusion In accordance with the standard screening process,primary medical staff should actively screen all the indicator cases with sputum bacteria results,and focus on family members aged 18-64 years old,which is an effective screening method to improve the detection rate of tuberculosis patients.
