中药泥灸联合擀筋棍刮痧在寒湿痹阻型肩周炎患者中的应用

Application of TCM Mud Moxibustion Combined with Massage Stick Scraping Therapy in Patients with Scapulohumeral Periarthritis Caused by Cold-Dampness Obstruction

目的探讨中药泥灸联合擀筋棍刮痧对改善寒湿痹阻型肩周炎的效果。方法选取2019年11月至2020年10月收治于北京中医药大学附属护国寺中医医院的86例寒湿痹阻型肩周炎患者作为研究对象,将其随机分为观察组(n=43)和对照组(n=43)。对两组患者均实施骨痹病中医护理方案,同时给予对照组患者单纯的中药泥灸治疗,在此基础上给予观察组擀筋棍刮痧治疗,每周2次。干预2周后,比较两组患者的肩关节功能Constant-Murley评分变化及临床疗效。结果干预2周后,观察组患者的临床治疗总有效率为95.35%,高于对照组的74.42%(P<0.05)。干预2周后,两组患者的肩关节活动度Constant-Murley评分均较之前有所提高,且观察组患者的Constant-Murley总分及肩关节疼痛、日常生活活动、关节活动度及肌力4个维度得分均高于对照组,差异具有统计学意义(P<0.05)。干预1个疗程后,观察组Constant-Murley各项评分及总分均优于对照组,差异有统计学意义(P<0.05)。结论中药泥灸联合擀筋棍刮痧能够有效缓解寒湿痹阻所致肩周炎患者的肩关节疼痛症状,促进其关节功能恢复,提高患者生活质量。

Objective To explore the effect of TCM mud moxibustion combined with massage stick scraping therapy on scapulohumeral periarthritis caused by cold-dampness obstruction.Methods A total of 86 patients with scapulohumeral periarthritis caused by cold-dampness obstruction admitted to Huguosi Hospital of TCM Affiliated to Beijing University of Chinese Medicine from November 2019 to October 2020 were selected as the subjects.They were randomly divided into the observation group(n=43) and the control group(n=43).Both groups were given TCM care services for bone impediment.Besides,the control group received TCM mud moxibustion,while the observation group received massage stick scraping therapy on this basis,twice a week.After 2 weeks of intervention,the changes in Constant-Murley Shoulder Outcome score and clinical efficacy were compared between the two groups.Results After 2 weeks of intervention,the overall response rate of clinical treatment in the observation group was 95.35%,higher than 74.42% in the control group(P<0.05).After 2 weeks of intervention,the score of Constant-Murley range of motion was higher than before in both groups;the Constant-Murley total score and the score of each 4 dimensions(pain,activities of daily living,range of motion and strength) in the observation group were higher than those in the control group,with a statistically significant difference(P<0.05).After a course of intervention,the Constant-Murley total score and the score of each dimension in the observation group were superior to those in the control group,with a statistically significant difference(P<0.05).Conclusion TCM mud moxibustion combined with massage stick scraping therapy can effectively relieve shoulder pain in patients with scapulohumeral periarthritis caused by cold-dampness obstruction,speed up the recovery of joint function and improve their quality of life.
