居家腹膜透析患者社区延续护理服务清单的构建

Construction of A Community Transitional Care Service List for Home-based Peritoneal Dialysis Patients

目的通过专家咨询构建居家腹膜透析患者社区延续护理服务清单,为社区开展居家腹膜透析护理提供参考。方法基于文献回顾及患者需求调查初步形成包含29项内容的腹膜透析患者社区延续性护理服务清单,并于2019年6月至2021年9月,选取16名腹膜透析相关专家进行了3轮咨询,对相关条目进行评价。结果3轮咨询的专家权威系数分别为0.80、0.83和0.86,肯德尔协调系数分别为0.378、0.202和0.417,最终专家意见趋同。经3轮咨询,最终形成包含自我护理培训、建档与随访、心理社会支持及个性化指导4大类、27项服务内容的居家腹膜透析患者社区延续护理服务清单。结论通过3轮专家咨询构建的居家腹膜透析患者社区延续护理服务清单兼顾了患者生理、心理、社会等多方面需求,可为社区卫生服务中心开展居家腹膜透析延续护理服务提供相应指导。

Objective To make a list of community transitional care services for home-based peritoneal dialysis patients through expert consultation,providing a reference for home-based peritoneal dialysis in communities.Methods A list of 29 items of community transitional care services for peritoneal dialysis patients was preliminarily made based on literature review and patient demand survey.From June 2019 to September 2021,a total of 16 peritoneal dialysis experts were selected for 3 rounds of consultation to evaluate the relevant items.Results For the 3 rounds of consultation,the authoritative coefficient was 0.80,0.83 and 0.86 and Kendall’s coefficient of concordance was 0.378,0.202 and 0.417,respectively,indicating consistent expert opinions.After 3 rounds of consultation,a list of community transitional care services for home-based peritoneal dialysis patients was finally made to contain 4 categories(self-care training,filing and follow-up,psychosocial support and personalized guidance) and 27 service items.Conclusion The list of community transitional care services for home-based peritoneal dialysis patients made through 3 rounds of expert consultation takes into account the physical,psychological,social and other needs of patients,providing corresponding guidance for community healthcare centers to offer transitional care services for home-based peritoneal dialysis patients.
