颈动脉内膜剥脱术治疗症状性颈内动脉狭窄的疗效观察

Effect of carotid endarterectomy on symptomatic internal carotid artery s tenosis

目的:研究用颈动脉内膜剥脱术(CEA)治疗症状性颈内动脉狭窄的疗效。方法:选取梧州市中医医院2018年12月至2021年1月接诊的80例症状性颈内动脉狭窄患者,采用随机数表法将其分为CEA组和颈动脉支架置入术(CAS)组(40例/组)。为CEA组患者采用CEA进行治疗,为CAS组患者采用CAS进行治疗,然后对比两组患者的疗效。结果:治疗后,CEA组患者病变颈内动脉相应供血区域脑组织的血流平均通过时间、脑灌注达峰时间均短于CAS组患者,其病变颈内动脉相应供血区域脑组织的局部脑血容量、局部脑血流量均大于CAS组患者,其简易神经状态检查量表的评分高于CAS组患者,P<0.05。两组患者术后脑血管并发症的发生率相比,P>0.05。结论:与采用CAS相比,用CEA治疗症状性颈内动脉狭窄的疗效较好,可显著改善患者的脑血流灌注,促进其认知功能的恢复,且安全性较高。

Objective:To study the efficacy of carotid endarterectomy(CEA)in the treatment of symptomatic internal carotid artery stenosis.Methods:A total of 80 patients with symptomatic internal carotid artery stenosis admitted to Wuzhou Hospital of Traditional Chinese Medicine from December 2018 to January 2021 were selected and divided into CEA group and CAS group(40 cases/group)by random number table method.CEA group was treated with CEA,CAS group was treated with CAS,and then the efficacy of the two groups was compared.Results:After treatment,the CEA group of lesions in patients with internal carotid artery blood supply corresponding regional average through time,cerebral blood flow to the brain tissue perfusion tmax is shorter than the CAS group of patients,the pathological changes of internal carotid artery blood supply region corresponding brain tissue local cerebral blood volume and local cerebral blood flow were greater than the CAS group of patients,its simple neural state examination scale score higher than that of CAS group of patients,(P<0.05).The incidence of postoperative cerebrovascular complications was compared between the two groups,P>0.05.Conclusion:Compared with CAS,CEA has better efficacy in the treatment of symptomatic internal carotid artery stenosis,which can significantly improve cerebral blood perfusion and promote the recovery of cognitive function,with high safety.
