鞘内泵注吗啡联合口服加巴喷丁治疗中重度癌性疼痛的效果

Effect of intrathecal pump morphine combined with oral gabapentin in the treatment of moderate and severe cancer pain

目的:探讨为中重度癌性疼痛患者鞘内泵注吗啡联合口服加巴喷丁进行治疗的效果。方法:选取2019年1月至2020年1月期间中国人民解放军联勤保障部队第九七〇医院收治的80例中重度癌性疼痛患者作为研究对象。随机将这些患者分为参照组和研究组。为参照组患者鞘内泵注吗啡进行治疗,为研究组患者鞘内泵注吗啡联合口服加巴喷丁进行治疗。然后观察两组患者癌性疼痛程度的视觉模拟评分法(VAS)评分、匹兹堡睡眠质量指数评分表(PSQI)的评分、补用镇痛药物的剂量及发生不良反应的情况。结果:治疗后2周及4周,研究组患者癌性疼痛程度的VAS评分低于参照组患者,P<0.05。治疗后,研究组患者的PSQI评分及不良反应的发生率均低于参照组患者,其补用镇痛药物的剂量少于参照组患者,P<0.05。结论:为中重度癌性疼痛患者鞘内泵注吗啡联合口服加巴喷丁进行治疗可有效地减轻其癌性疼痛的程度,提高其睡眠质量,减少其补用镇痛药物的剂量,且安全性较高。

Objective:To investigate the effect of intrathecal pumping morphine combined with oral gabapentin in the treatment of moderate and severe cancer pain.Methods:A total of 80 patients with moderate to severe cancer pain admitted to the 97th Hospital of the Joint Logistic Support Force of the Chinese People’s Liberation Army from January 2019 to January 2020 were selected as the study subjects.These patients were randomly divided into control group and study group.Patients in the control group were treated with intrathecal infusion of morphine,and patients in the study group were treated with intrathecal infusion of morphine combined with oral gabapentin.Then,visual analogue scale(VAS)score,Pittsburgh Sleep Quality Index(PSQI)score,the dosage of analgesic drugs and the occurrence of adverse reactions were observed in 2 groups.Results:At 2 and 4 weeks after treatment,VAS score of cancer pain in the study group was lower than that in the reference group,P<0.05.After treatment,the PSQI score and the incidence of adverse reactions of patients in the study group were lower than those in the reference group,and the dosage of analgesics supplement was lower than that in the reference group,P<0.05.Conclusion:Intrathecal injection of morphine combined with oral gabapentin can effectively reduce the degree of cancer pain,improve sleep quality and reduce the dosage of analgesic drugs for moderate a nd severe cancer pain,with high safety.
