三棱的临床应用及其用量探究

通过搜集古代医籍及现代医家临床应用文献,总结出三棱具有以下特点:三棱的临床汤剂用量范围为3〜60g,常用量为4.5〜9g,丸散剂常用量为0.75〜11.25 g。临床根据不同的疾病,证型,症状可选择不同的剂量,如行气化瘀,治疗肺胀、咳血等肺系疾病以及瘀血阻滞,心脉痹阻所致的心悸,应用剂量为18〜30 g(汤剂);消癥止痛,治疗以妇科为主的疾病(卵巢囊肿,子宫肌瘤),常用量6〜30g(汤剂),4.68〜11.25g(丸剂);化瘀消积,治疗新生儿黄疽兼肝脾肿大、小儿疮积、食滞、血小板减少等,常用量10〜15 g(汤剂),0.75g(散剂);行气化瘀,治疗男子不育症、阳痿、黄褐斑等,常用量6〜30 g(汤剂);破血行气,治疗脏器纤维化、肝硬化、反复发作性坏死性淋巴结炎症等疾病,常用量3〜60g(汤剂)。根据疾病、证型及症状,配伍相应中药,如行气化瘀,用三棱配伍金荞麦、枳实、厚朴、薤白等;消癥止痛,配伍莪术、紫石英、淫羊藿、花椒、川芎、丹参;化瘀消积,配伍莪术、旱莲草、青皮;行气化滞,配伍萆薢、黄芩、熟地黄、莪术等;破血行气,配伍郁金、三七、赤芍、莪术、猫爪草等。

By collecting ancient medical records and clinical application literature of modern physicians,it is concluded that common buried rubber has the following characteristics:Its clinical dosage range in decoction is 3-60 g,and its common dosage range is 4.5-9 g,while in pills or powder 0.75-11.25 g.Its clinical dosage should be selected according to different diseases,syndrome types or symptoms.For instance,when used for activating qi-flowing to remove blood stasis in the treatment of lung diseases such as lung distension and hemoptysis,as well as palpitation caused by syndromes of static blood obstruction and stagnation,blockade of heart vessels,its applied dosage is 18-30 g (in decoction).For resolving lumps and relieving pain to treat gynecological diseases (ovarian cysts,uterine fibroids),its usual dosage range is 6-30 g (in decoction) or 4.68-11.25 g (in pills).For removing blood stasis and resolving masses to treat neonatal jaundice with hepatosplenomegaly,sores or accumulation in children,food retention,thrombocytopenia,etc.its commonly used dosage is 10~15 g (in decoction),or 0.75 g (in powder).For promoting qi circulation and removing blood stasis to treat male infertility,impotence,chloasma,etc.,its usual dosage is 6-30 g (in decoction).For activating blood circulation and qi-flowing in the treatment of organ fibrosis,liver cirrhosis,recurrent necrotizing lymphadenitis and other diseases,its commonly used dosage range is 3-60 g (in decoction).According to varying diseases,syndrome types or symptoms,common buried rubber is compatible with corresponding Chinese medicines.For example,for activating qi-flowing to remove blood stasis,it is often combined with golden buckwheat rhizome,immature orange fruit,officinal magnolia bark,longstamen onion bulb,etc.For resolving lumps and relieving pain,it is often combined with zedoray rhizome,fluorite,epimedium herb,pricklyash peel,Sichuan lovage rhizome and danshen root.For removing blood stasis and resolving masses,it is often combined with zedoray rhizome,herba ecliptae and immature tangerine peel.For promoting qi circulation and removing blood stasis,it is often combined with yam rhizome,baical skullcap root,prepared rehmannia root,zedoray rhizome,etc.For activating blood circulation and qi-flowing,it is often combined with turmeric root tuber,sanqi,peony root,zedoray rhizome,catclaw buttercup root,etc.
