阿立哌唑和利培酮联合多奈哌齐治疗阿尔茨海默病精神症状的疗效分析

Clinical analysis of aripiprazole and risperidone combined with Donepezil in the treatment of psychotic symptoms in Alzheimer’s disease

目的探讨阿立哌唑和利培酮联合多奈哌齐治疗阿尔茨海默病精神症状的疗效。方法选择2018年03月至2020年04月在黄河中心医院神经内科就诊的88例有精神症状的阿尔茨海默病患者为研究对象,以随机数字表法将患者分为观察组(n=44)和对照组(n=44),观察组患者给予阿立哌唑联合多奈哌齐治疗,对照组患者给予利培酮联合多奈哌齐治疗,对照两组患者精神症状、认知功能及不良反应情况。结果治疗后,两组患者BEHAVE-AD评分均有明显下降,差异有统计学意义(P<0.05);但两组比较,差异无统计学意义(P>0.05)。治疗后,两组患者MMSE评分均有所改善,但差异无统计学意义(P>0.05),两组比较差异亦无统计学意义(P>0.05);观察组静坐不能、肌强直、震颤、嗜睡等不良反应发生率明显低于对照组,差异有统计学意义(P<0.05)。结论与利培酮联合多奈哌齐相比,阿立哌唑联合多奈哌齐治疗阿尔茨海默病,可有效改善精神症状,并具有较高的安全性,但对患者认知功能影响不大。

Aim To investigate the efficacy of aripiprazole and risperidone combined with Donepezil in the treatment of psychotic symptoms in Alzheimer’s disease.Methods 88 cases of Alzheimer’s disease with psychotic symptoms admitted to the Department of Neurology,Yellow River central hospital from March 2018 to April 2020 were enrolled.The patients were divided into two groups by random number table.Patients in the observation group were treated with aripiprazole combined with donepezil,and patients in the control group were treated with risperidone combined with donepezil.The psychiatric symptoms,cognitive function and adverse reactions of the twu groups were compared.Results After treatment,the BEHAVE-AD scores of the two groups of patients decreased significantly,and the difference was statistically significant(P<0.05);but the difference between the two groups was not statistically significant(P>0.05).After treatment,the MMSE scores of the two groups were improved,but the difference was not statistically significant(P>0.05),and the difference between the two groups was not statistically significant(P>0.05).The incidence of adverse reactions such as akathisia,muscle rigidity,tremor,and lethargy in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Compared with risperidone combined with donepezil,aripiprazole combined with donepezil in the treatment of Alzheimer’s disease can effectively improve mental symptoms,and has higher safety,but has little effect on patients’cognitive function.
