右美托咪定复合罗哌卡因在超声引导下臂丛神经阻滞镇痛中的疗效及VAS评分影响分析 被引量:7

Effect of dexmedetomidine combined with ropivacaine in ultrasound-guided brachial plexus block analgesia and its influence on VAS score

摘要 目的探究在超声引导下臂丛神经阻滞镇痛中应用右美托咪定复合罗哌卡因的效果。方法随机将2018年3月至2019年8月本院70例行超声引导下臂丛神经阻滞患者分为对照组(35例,应用罗哌卡因)、实验组(35例,应用右美托咪定复合罗哌卡因)。对比两组VAS评分(疼痛评分)、麻醉效果、感觉阻滞起效时间、运动阻滞起效时间、镇痛持续时间及不良反应发生率。结果实验组麻醉优良率(97.14%)高于对照组(80.00%),P<0.05,且实验组VAS评分较对照组更低,P<0.05;实验组运动阻滞起效时间、感觉阻滞起效时间较对照组更短,且镇痛持续时间较对照组更长,P<0.05;实验组不良反应发生率和对照组无显著差异,P>0.05。结论在超声引导下臂丛神经阻滞镇痛中应用右美托咪定复合罗哌卡因更有助于缩短感觉阻滞和运动阻滞起效时间,增强镇痛效果,且镇痛维持时间更长,安全性较好。 Objective To explore the effect of dexmedetomidine combined with ropivacaine in ultrasound-guided brachial plexus block analgesia.Methods 70 patients with ultrasound-guided brachial plexus block in our hospital from March 2018 to August 2019 were randomly divided into control group(35 cases,ropivacaine)and experimental group(35 cases,dexmedetomidine combined with ropivacaine).VAS score(pain score),anesthetic effect,onset time of sensory block,onset time of motor block,duration of analgesia and incidence of adverse reactions were compared between the two groups.Results The excellent and good rate of anesthesia in the experimental group(97.14%)was higher than that in the control group(80.00%)(P<0.05),and the VAS score of the experimental group was lower than that of the control group(P<0.05);the onset time of motor block and sensory block in the experimental group was shorter than that in the control group,and the duration of analgesia was longer than that in the control group(P<0.05);there was no significant difference in the incidence of adverse reactions between the experimental group and the control group(P>0.05).Conclusion Dexmedetomidine combined with ropivacaine in ultrasound-guided brachial plexus block analgesia can shorten the onset time of sensory block and motor block,enhance the analgesic effect,and maintain analgesia for a longer time with better safety.
