摘要
目的临床观察阿立哌唑对抗精神病药物导致女性分裂症患者高泌乳素血症的影响。方法选取了第二师焉耆医院精神心理科自2018年1月至2019年12月收治的58例分别使用利培酮、氨磺必利、奥氮平治疗4周后出现高泌乳血症的女性精神分裂症患者,随机分为干预组和对照组各29例,在原治疗方案基础上,干预组合并阿立哌唑口腔崩解片5 mg/d,与对照组随访8周;分别于第4、第8周末检测泌乳素水平,用PANSS量表和TESS量表评定两组的治疗效果和药物副作用。结果干预组泌乳素水平于第4周末开始下降,第8周末明显下降,甚至有3例(10.7%)恢复正常;对照组泌乳素水平经过8周的随访较入组时均有升高。两组间差异有统计学意义(P<0.05);两组PANSS各项评分及总分和TESS评定无明显统计学差异(P>0.05)。结论合并小剂量阿立哌唑口腔崩解片可降低抗精神病药物引起的高泌乳素水平。
Objective Clinical observation of the effect of aripiprazole on hyperprolactinemia in women with schizophrenia caused by antipsychotics.Method Selected 58 female patients with schizophrenia who were admitted to the Department of Psychiatry in Yanqi Hospital of the Second Xinjiang Production and Construction Corps Division from January 2018 to December 2019.These patients who developed hyperlactemia after 4 weeks of treatment with risperidone,amisulpride,and olanzapine were randomly divided into intervention group and control group with 29 cases each.On the basis of the original treatment plan,the intervention group combined with aripiprazole orally disintegrating tablets 5 mg/d,followed up with the control group for 8 weeks;The levels of prolactin were measured at the 4 th and 8 th weekends,and the PANSS scale and TESS scale were used to evaluate the treatment effect and drug side effects of the two groups.Result The prolactin level of the intervention group began to decline at the 4 th weekend,and significantly decreased at the 8 th weekend,and even3 cases(10.7%)returned to normal;The level of prolactin in the control group was higher after 8 weeks of follow-up than when it was enrolled.The difference between the two groups was statistically significant(P<0.05);there was no statistically significant difference between the two groups in PANSS scores and total scores and TESS assessment(P>0.05).Conclusion Combined with low-dose aripiprazole orally disintegrating tablets can reduce the high prolactin levels caused by antipsychotics.
