甲状腺微小乳头状癌高侵袭性的解读被引量：4

摘要世界卫生组织定义肿瘤最大径≤1 cm 的甲状腺乳头状癌(papillary thyroid carcinoma, PTC )为甲状腺微小乳头状癌(papillary thyroid microcarci-noma, PTMC)[1].近年来PTMC占新发病例50%以上[2].PTMC在目前甲状腺癌诊治工作中占有很大权重.大部分PTMC呈惰性特征,其15年疾病特异性生存率为99%,复发率为5%[3].但仍有少部分PTMC具有高侵袭性特征,侵出甲状腺被膜或侵犯周围重要组织,出现中央区、颈侧区淋巴结转移,甚至远处转移.高侵袭性PTMC是造成疾病进展和死亡的主要原因,在临床工作中需充分认知并规范处理,但目前尚无明确的临床病理指标评估或预测PTMC的高侵袭性.本文从高侵袭性的界定、相关因素、分子特征对PTMC的高侵袭性进行解读,以期为临床治疗提供策略.
