全程护理干预在老年患者无痛胃镜检查中效果分析

Effect of whole-course nursing intervention on painless gastroscopy in elderly patients

目的探讨全程护理干预在老年患者无痛胃镜检查中的效果。方法选择2018年7月~2019年11月本院行无痛胃镜检查老年患者98例,用随机数表法分2组,各49例。对照组施行常规护理,观察组施行全程护理。比较2组患者检查时间及检查期间心率、收缩压、麻醉药物使用剂量及呼吸频率,记录护理前后焦虑自评量表(SAS)评分、抑郁自评量表(SDS)评分变化,统计护理期间并发症发生情况及患者满意率。结果观察组患者检查时间[(22.10±3.15)min]短于对照组[(23.69±4.02)min],观察组患者检查期间心率为(98.71±12.10)次/min、收缩压为(121.13±10.79)mmHg、呼吸频率为(17.13±2.54)次/min、麻醉药物使用剂量为(28.10±3.22)mL均低于对照组[(108.46±10.31)次/min、(132.62±9.79)mmHg、(21.85±2.33)次/min、(34.18±2.97)mL](P<0.05);护理后,2组患者的SAS评分、SDS评分均降低,且观察组[(23.52±2.79)分、(21.09±2.79)分]低于对照组[(28.14±2.12)分、(25.71±2.99)分](P<0.05);护理期间,观察组并发症发生率(8.16%)低于对照组(24.49%),(P<0.05);观察组患者的满意率为(95.52%),显著高于对照组的(81.63%),(P<0.05)。结论无痛胃镜检查中给予老年患者全程护理,可有效减少内径对生理状态的影响,降低麻醉药物使用剂量,消除负面情绪,缩短检查时间,并降低检查并发症发生率,提高患者满意率,值得临床推广。

Objective To explore the effect of whole course nursing intervention in elderly patients with painless gastroscopy.Methods 98 elderly patients undergoing painless gastroscopy in our hospital from July 2018 to November 2019 were randomly divided into two groups,with 49 cases in each group.The control group was given routine nursing,while the observation group was treated with whole course nursing.The examination time,heart rate,systolic blood pressure,anesthetic dosage and respiratory rate were compared between two groups.The changes of SAS score and SDS score before and after nursing were recorded.The incidence of complications and satisfaction rate of patients during nursing were analyzed.Result The examination time of patients in the observation group[(22.10±3.15)min]was shorter than that of the control group[(23.69±4.02)min].During the examination of patients in the observation group,the heart rate was(98.71±12.10)beats/min and the systolic blood pressure was(121.13±10.79)mm Hg,respiratory rate of(17.13±2.54)times/min,and anesthetic dose of(28.10±3.22)m L were all lower than those in the control group[(108.46±10.31)times/min,(132.62±9.79)mm Hg,(21.85±2.33)times/min,(34.18±2.97)m L](P<0.05).After nursing,the SAS score and SDS score of the two groups were decreased,and the observation group[(23.52±2.79)points,(21.09±2.79)points]was lower than that of the control group[(28.14±2.12)points,(25.71±2.99)points](P<0.05).During the nursing,the incidence of complications in the observation group(8.16%)was lower than that in the control group(24.49%),(P<0.05).The satisfaction rate of the observation group(95.52%)was significantly higher than that of the control group(81.63%)(P<0.05).Conclusion Whole-course nursing during painless gastroscopy effectively reduce the influence of inner diameter on physiological state,reduce the dosage of narcotic drugs,eliminate negative emotions,shorten the examination time,reduce the incidence of complications and improve the satisfaction rate of patients,which is worthy of clinical promotion.
