1.5 T核磁共振技术在膝关节损伤诊断中的应用分析 被引量:3

摘要 目的探究1.5 T核磁共振技术在膝关节损伤诊断中的应用价值。方法60例膝关节损伤患者,随机分为对照组和实验组,各30例。对照组患者给予常规CT进行诊断,实验组患者给予1.5 T核磁共振技术进行诊断。对比两组患者损伤类型与诊断检出率,并分析核磁共振诊断影像学表现。结果对照组诊断检出率为76.7%,其中半月板损伤5例,韧带损伤4例,骨质损伤3例,关节腔积液11例;实验组诊断检出率为100.0%,其中半月板损伤10例,韧带损伤6例,骨质损伤3例,关节腔积液11例;实验组诊断检出率高于对照组,差异具有统计学意义(P<0.05)。半月板损伤的核磁共振影像学表现:伤处呈现低信号阴影,并且阴影内存在放射状、线状、球状或椭圆形裂隙样阴影,半月板区域呈现不规则或现状的高信号;韧带损伤的核磁共振影像学表现:T1加权成像(T1WI)加权信号呈现为高低混合信号或者低信号,T2加权成像(T2WI)加权信号呈现为高信号;伤处主要会呈现扁平条带状的低信号阴影,边缘会出现波浪状,韧带肿胀的部位会出现不规则高信号,外形不规则,韧带不规则增粗,并且与周围组织分界线比较模糊;骨质损伤的核磁共振影像学表现:患处存在异常的网络状信号,边界比较模糊,T2WI加权信号呈现稍微高信号或等信号,T1WI加权信号主要呈现为低信号,并且内部存在小片状的高信号;关节腔积液的核磁共振影像学表现:患处呈现条状低信号,边缘比较模糊或者呈现波浪形,部分存在分层情况或高信号,T2WI加权信号呈现为高信号,而T1WI加权信号呈现为高低混合信号或低信号。结论对膝关节损伤患者采用1.5 T核磁共振技术进行诊断,能够显著提高诊断准确率,明确患者膝关节损伤类型,具有较高的安全性与实用性,临床应用价值较高。 Objective To explore the value of applying 1.5 T magnetic resonance imaging(MRI)in the diagnosis of knee injury.Methods 60 patients with knee injury were randomly divided into experimental group and control group,with 30 cases in each group.Patients in the control group were diagnosed by routine CT,while patients in the experimental group were diagnosed with 1.5 T MRI.Injury type and diagnostic detection rate were compared between the two groups.The MRI diagnostic imaging findings were analyzed.Results The diagnostic detection rate of the control group was 76.7%,wherein 5 cases of meniscus injury,4 cases of ligament injury,3 cases of bone injury and 11 cases of joint effusion;while the diagnostic detection rate of the experimental group was 100.0%,comprising 10 cases of meniscus injury,6 cases of ligament injury,3 cases of bone injury and 11 cases of joint effusion.The diagnostic detection rate of the experimental group was higher than that of the control group,and the difference was statistically significant(P<0.05).The MRI manifestations of meniscus injury were low signal shadow,radial,linear,spherical or oval fissure like shadow,and irregular or current high signal in meniscus area.The main MRI manifestations of ligament injury were T1 weighted imaging(T1WI)weighted signal showed high-low mixed signal or low signal,and T2 weighted imaging(T2WI)weighted signal showed high signal;and the injury location mainly presented flat strip low signal shadow with wavy edges,while at the swollen part of ligament,there were irregular high signal with irregular shape,irregular thickening of ligament,and fuzzy boundary with surrounding tissue.After bone injury,the MRI findings showed that there were abnormal network signals in the affected area,with the boundary was fuzzy.T2WI weighted signals appeared with slightly high signals or equal signals,while T1WI weighted signals mainly showed as low signals,wherein there were small pieces of high signals.The MRI manifestations of joint cavity effusion were strip-shaped low signal,with fuzzy or wavy edge,and some of them had stratification or high signal.The T2WI weighted signal appeared as high signal,while the T1WI weighted signal appeared as mixed signal of high signal and low signal or low signal.Conclusion The application of 1.5 T MRI technology in the diagnosis of knee injury may significantly improve the diagnostic accuracy and identify the type of knee injury,which has high safety and practicability.Therefore,it has high clinical application value.
