摘要
目的了解郑州市2020年健康人群乙型病毒性肝炎(乙肝)血清流行率。方法采用多阶段随机抽样法,在郑州市5个县区选择0-79岁健康人群,采集血清标本,分别采用酶联免疫吸附试验和化学发光免疫试验检测乙肝表面抗原(HBsAg)和乙肝表面抗体(HBsAb),分析血清流行率。结果调查对象HBsAg、HBsAb总阳性率分别为0.61%(10/1 650)、69.70%(1 150/1 650)。0-6岁、7-19岁、20-39岁、40-59岁、60-79岁年龄组HBsAg阳性率分别为0.00%、0.33%、0.67%、0.66%、3.40%;HBsAb阳性率分别为82.83%、46.73%、80.67%、64.69%、52.38%。末剂次乙肝疫苗接种后<3年、3-5年、6-8年、≥9年调查对象的HBsAg阳性率分别为0.00%、0.00%、0.00%、1.28%,HBsAb阳性率分别为93.58%、65.92%、60.47%、52.56%。结论郑州市2020年健康人群特别是小年龄儿童HBsAg携带率显著下降,HBsAb随乙肝疫苗接种后时间推移而衰减;建议探讨青少年和成人乙肝免疫策略。
Objective To determine hepatitis B marker seroprevalences in a healthy population of Zhengzhou city in 2020.Methods We used a multi-stage random sampling method to recruit 0-79-year-old healthy people in 5 counties of Zhengzhou. We tested serum samples for hepatitis B surface antigen(HBsAg) and hepatitis B surface antibody(HBsAb) using enzyme-linked immunosorbent assays and chemiluminescence immunoassays to determine hepatitis B sero-positivity.Results Overall HBsAg and HBsAb positivity rates were 0.61%(10/1 650) and 69.70%(1 150/1 650), respectively. HBsAg positivity among 0-6-, 7-19-, 20-39-, 40-59-, and 60-79-year-olds were 0.00%, 0.33%, 0.67%, 0.66%, and 3.40%, with HBsAb positivity being 82.83%, 46.73%, 80.67%, 64.69%, and 52.38%. HBsAg prevalence for subjects who had received their last dose of hepatitis B vaccine <3, 3-5, 6-8, and ≥9 years previously were 0.00%, 0.00%, 0.00%, and 1.28%, with HBsAb positivity rates being 93.58%, 65.92%, 60.47%, and 52.56%.Conclusions HBsAg carriage decreased significantly in a healthy population of Zhengzhou in 2020, especially among younger children. HBsAb positivity decreased with increasing time since hepatitis B vaccination. We suggest exploring hepatitis B immunization strategies for adolescents and adults.
