腹腔镜前列腺癌根治术治疗早期局限性前列腺癌的效果分析

Analysis of the effect of laparoscopic radical prostatectomy for early localized prostate cancer

目的分析腹腔镜前列腺癌根治术治疗早期局限性前列腺癌的临床效果。方法回顾性分析2017年1月至2020年1月在我院治疗的60例早期局限性前列腺癌患者的临床资料,按治疗方式分为对比组(给予经耻骨后前列腺癌根治术治疗)和研究组(给予腹腔镜前列腺癌根治术治疗),每组各30例。比较两组治疗前后的尿流动力学指标[逼尿肌在最大尿流率时所呈现出的压力(Pdetat MFR)、残余尿量(RUV)、最大尿流率(MFR)、充盈期膀胱顺应性值(BC)]、国际前列腺症状评分(IPSS)与生存质量量表评分(SF-36);比较两组术中出血量、手术时间、胃肠功能恢复时间、住院时间、并发症发生率、生化复发率、尿把控正常率。结果治疗前,两组的Pdetat MFR、RUV、MFR、BC与IPSS、SF-36评分比较,差异无统计学意义(P>0.05)。治疗后,研究组Pdetat MFR、MFR和SF-36评分高于对比组,而RUV、BC和IPSS评分低于对比组(P<0.05);研究组术中出血量少于对比组,而手术时间、胃肠功能恢复时间、住院时间均短于对比组(P<0.05);研究组并发症发生率、生化复发率低于对比组,尿把控正常率高于对比组(P<0.05)。结论为早期局限性前列腺癌患者进行腹腔镜前列腺癌根治术治疗,疗效显著,值得推广应用。

Objective To analyze the clinical effect of laparoscopic radical prostatectomy for patients with localized prostate cancer(LPC)at early stage.Methods The clinical data of 60 patients with early LPC at early stage treated in our hospital from January 2017 to January 2020 were retrospectively analyzed.According to the treatment methods,they were divided into the control group(retropubic radical prostatectomy)and the study group(laparoscopic radical prostatectomy),with 30 cases in each group.The urodynamic indexes[detrusor pressure at maximum urinary flow rate(Pdet at MFR),residual urine volume(RUV),maximum urinary flow rate(MFR),bladder compliance(BC)at filling stage],International Prostate Symptom Score(IPSS)and Short Form-36 questionnaire(SF-36)before and after treatment were compared between the two groups.The intraoperative blood loss,the operation time,the time needed for gastrointestinal function recovery,the hospitalization time,the complication rate,the biochemical recurrence(BCR)rate and the normal rate of urine control were compared between the two groups.Results Before treatment,there were no differences in Pdet at MFR,RUV,MFR,BC,IPSS and SF-36 scores between the two groups(P>0.05).After treatment,Pdet at MFR,MFR and SF-36 score were higher in the study group than those in the control group,while RUV,BC and IPSS score were lower in the study group than those in the control group(P<0.05).The amount of intraoperative hemorrhage was less in the study group than that in the control group,while the operation time,the time needed for gastrointestinal function recovery and hospitalization time were shorter in the study group than those in the control group(P<0.05).The incidence of complications and BCR were lower in the study group than those in the control group,and the normal rate of urine control in the study group was higher than that in the control group(P<0.05).Conclusion Laparoscopic radical prostatectomy is significantly effective for patients with LPC at early stage.Thus,it is worthy of popularization and application.
