公立医院制度性诱导需求的资金流转量化模型构建

摘要 目的在系统分析的基础上,构建公立医院制度性诱导需求的资金流转量化模型。方法首先,结合系统结构和反馈回路,绘制公立医院医疗费用增长系统流图。然后,确定模型中的变量含义以及变量之间的量化关系。按照提出方法、数据模拟、反复论证、完善方法的思路完善模型量化方法。结果模型分为3个子模块:一是医疗费用流转模块,关注系统结构的内部流转运作模式;二是医疗费用增长模块,关注驱动因素对医疗费用增长的推动作用;三是政策干预模拟模块,关注医疗费用增长的政策遏制效果。结论实现流转模块10类水平变量、4类速率变量的量化关系,实现增长模块3类水平变量和4类辅助变量的量化关系,实现政策干预模块1类水平变量和6类辅助变量的量化关系。综合3个子模型,形成我国公立医院医疗费用增长的系统动力量化模型。 Objective Construct a quantitative model of the capital flow of institutional induced demand in public hospitals on the basis of systematic analysis. Methods Firstly,the system flow chart of medical cost growth in public hospitals was drawn by combining system structure and feedback loop. Then,the meaning of variables in the model and the quantitative relationship between variables were determined. The model quantization method was improved according to the ideas of the proposed method,data simulation,repeated demonstration and method improvement. Results The model was divided into three sub-modules:the medical cost circulation module,which focuses on the internal circulation mode of the system structure;the module on medical cost growth,focusing on the driving factors of medical cost growth;the simulation module of policy intervention,focusing on the effect of curbing the growth of medical costs. Conclusion It realizes the quantitative relationship of 10 level variables and 4 rate variables in the flow module;the quantitative relationship between 3 types of horizontal variables and 4 types of auxiliary variables in the growth module;and the quantitative relationship between a class of horizontal variables and six auxiliary variables in the policy intervention module. By integrating 3 sub-models,the system dynamic quantitative model of medical cost growth in public hospitals in China is formed.
