摘要
目的了解2015年和2018年贵州省县级医院管理水平发展情况。方法采用县级医院机构调查表及经翻译和本土化修订后的发展中国家医院管理调查问卷,分别于2015年和2018年,通过现场访谈定量测量贵州省73家县级医院的管理水平,使用配对t检验分析医院管理水平的前后变化。结果贵州省县级医院规模和管理者素质有所提升;医院整体管理平均得分从2.55分提升至2.69分,其中监督管理和目标管理的提升差异有统计学意义(P<0.05)。公立综合性医院、公立中医院、民营医院的管理得分及变化趋势存在差别。结论以贵州省为代表的我国西部县级医院管理水平呈良性发展状态,特别是公立综合性医院的目标管理和监督管理方面有明显提高。此外,县级医院规模扩大和管理者素质提高也是影响医院管理水平的重要理论因素。
Objective To understand the management status of county-level hospitals in Guizhou Province and analyze its development in 2015 and 2018. Methods Using the county-level hospital institution questionnaire and the translated and localized version of the Development World Management Survey for hospitals in developing countries,73 county-level hospitals’ management were measured through on-site interviews respectively in Guizhou Province in 2015 and 2018. The paired t-test was applied to analyze the changes of management before and after reform. Results The scale of county-level hospitals and the quality of managers have been improved in Guizhou Province.The overall hospital management(2.55,2.69) has been improved,including two significantly improved dimensions of monitoring management and target management. There were differences in management scores and trends among public general hospitals,public traditional Chinese medicine hospitals and private hospitals. Conclusion The management of county-level hospitals in western China represented by Guizhou Province is in a healthy of public general hospital state of development,especially the significant development in target management and monitoring management. In addition,the expansion of the scale and the improvement of the quality of managers were also important theoretical factors affecting the management of hospitals.
