765例生育二胎妇女产后抑郁发生情况及其影响因素研究

Study on the incidence of postpartum depression and its influence factor in 765second-born women

目的了解生育二胎妇女产后抑郁现状及其影响因素,为有效地干预提供依据。方法使用孕产妇一般情况调查表、社会支持量表、状态-特质焦虑量表以及爱丁堡产后抑郁量表,以整群抽样的方法,抽取在武汉市5所医院住院分娩的765名妇女,调查育妇产后抑郁发生情况及影响因素。结果产后抑郁发生率为19.22%;产妇无职业、婴儿的健康问题、产前特质-状态焦虑、睡眠质量差、婆家人照顾产褥期是产后抑郁有害因素;婴儿性别与期望一致、主观支持、支持利用度、一孩年龄≥6岁以及由月子中心照顾产褥期是产后抑郁保护性因素。结论二胎产妇产后抑郁的影响因素更为复杂,预防产后抑郁的发生需要产妇及其家人在孕前做好规划和准备,充分评估自身的情况,及时排查危险性因素,注意孕期保健和产褥期的照顾。

Objective To understand the status and influencing factors of postpartum depression in second-born women,and to provide evidence for effective intervention.Methods A total of 765 women who were hospitalized and delivered in 5 hospitals in Wuhan were investigated by using the general situation questionnaire,social support scale,State-Trait Anxiety Scale and Edinburgh postpartum depression scale.Results The incidence of postpartum depression was 19.22%.maternal unemployment,infant health problems,prenatal trait-state anxiety,poor sleep quality,and Husband’s family care during puerperium are positively correlated with postpartum depression.Babies’gender was consistent with expectations,subjective support and support utilization,age of older children(≥6 years old),and care of puerperium by the month center are significantly negatively correlated with postpartum depression.Conclusion The influencing factors of postpartum depression in second-born women are more complex.Preventing postpartum depression requires pregnant women and their families to plan and prepare well before pregnancy,fully assessing their own situation,timely checking investigate risk factors,and to pay attention to prenatal care and puerperium care.
