低相对分子质量肝素(LMWH)对重症急性胰腺炎(SAP)治疗效果及凝血功能的影响

The Effect of Low Molecular Weight Heparin(LMWH)on the Treatment Effect of Severe Acute Pancreatitis(SAP)and Blood Coagulation Function

目的探讨重症急性胰腺炎患者应用低相对分子质量肝素的临床治疗效果,及对患者凝血功能的影响。方法方便选取2019年12月—2021年5月该院收治的重症急性胰腺炎患者76例为研究对象,采用单盲分组方式按1∶1的比例分为对照组与研究组。对照组施以常规治疗,研究组施以常规治疗联合低相对分子质量肝素,对两组患者临床治疗效果、炎症因子水平及凝血功能进行比较。结果研究组患者临床治疗总有效率为94.74%,显著高于对照组,差异有统计学意义(χ^(2)=5.208,P=0.022);治疗后,研究组患者炎症因子水平与凝血功能方面的TNF-α、CRP、IL-6、D-D、APTT、PT分别为(3.17±0.33)ng/mL、(60.83±17.26)mg/mL、(54.39±6.27)ng/mL、(0.91±0.23)mg/mL、(45.78±5.37)s、(17.14±2.33)s,各项指标均优于对照组,差异有统计学意义(t=3.978、5.754、2.098、18.365、4.490、5.260,P<0.001)。结论应用低相对分子质量肝素对重症急性胰腺炎患者展开临床治疗,能够改善患者炎症因子水平,提升患者临床治疗效果,对于患者凝血功能的调节具有积极效果。

Objective To explore the clinical therapeutic effect of low molecular weight heparin in patients with severe acute pancreatitis,and the effect on the coagulation function of patients.Methods A total of 76 patients with severe acute pancreatitis admitted to the hospital from December 2019 to May 2021 were conveniently selected as the research objects.The single-blind grouping method was divided into a control group and a study group at a ratio of 1:1.The control group received conventional treatment,and the study group received conventional treatment combined with low molecular weight heparin.The clinical treatment effect,inflammatory factor levels and blood coagulation function of the two groups were compared.Results The total effective rate of clinical treatment in the study group was 94.74%,which was significantly higher than that in the control group,and the difference was statistically significant(χ^(2)=5.208,P=0.022);after treatment,the level of inflammatory factors and coagulation function in the study group were TNF-α,CRP,IL-6,DD,APTT,PT were(3.17±0.33)ng/mL,(60.83±17.26)mg/mL,(54.39±6.27)ng/mL,(0.91±0.23)mg/mL,(45.78±5.37)s,(17.14±2.33)s,all indicators were better than the control group,the difference was statistically significant(t=3.978,5.754,2.098,18.365,4.490,5.260,P<0.001).Conclusion The application of low molecular weight heparin for clinical treatment of patients with severe acute pancreatitis can improve the level of inflammatory factors in the patients,enhance the clinical treatment effect of the patients,and have a positive effect on the regulation of the coagulation function of the patients.
