沙库巴曲缬沙坦对慢性心力衰竭患者合并肾功能不全的临床疗效分析

Analysis of the Clinical Efficacy of Sacubitril and Valsartan in the Treatment of Chronic Heart Failure Patients with Renal Insufficiency

目的研究分析慢性心力衰竭(CHF)并肾功能不全患者采取沙库巴曲缬沙坦治疗后的临床效果。方法便利选取2019年9月—2020年10月该院接诊的86例CHF并肾功能不全患者作为研究对象,将2019年9月—2020年3月收治的患者采用依那普利治疗为对照组(n=43),2020年4—10月收治的患者采用沙库巴曲缬沙坦治疗为观察组(n=43),观察并比较两组治疗效果。结果观察组的总有效率为97.67%,远高于对照组的79.07%,差异有统计学意义(χ^(2)=7.242,P<0.05)。治疗前,组间各指标差异无统计学意义(P>0.05);治疗后1、3、6个月,观察组的LVEF、LVEDD和NT-proBNP均优于对照组,差异有统计学意义(t=2.224、2.228、3.292、4.131、4.394、3.831、3.064、2.511、2.128,P<0.05)。治疗后,观察组BUN和Scr指标分别为(8.35±1.02)mmol/L、(110.49±10.67)μmol/L均低于对照组(9.01±0.98)mmol/L、(116.13±10.15)μmol/L,差异有统计学意义(t=3.060、2.511,P<0.05)。结论应用沙库巴曲缬沙坦治疗疗效更为突出,能有效缓解心功能不全,改善肾功能,临床应用价值高。

Objective To study and analyze the clinical effects of sacubitril and valsartan in patients with chronic heart failure(CHF)and renal insufficiency.Methods Convenient selected 86 patients with CHF and renal insufficiency admitted in the hospital from September 2019 to October 2020 as the study objects,and the control group was treated with enalapril from September 2019 to March 2020(n=43),the observation group(n=43)was treated with sacubitril and valsartan from April 2020 to October 2020,and the treatment effects between the two groups was observed and compared.Results The total effective rate of the observation group was 97.67%,which was much higher than the 79.07%of the control group,the difference was statistically significant(χ^(2)=7.242,P<0.05).Before treatment,there was no significant difference in indicators between the groups(P>0.05);1,3,and 6 months after treatment,LVEF,LVEDD and NT-proBNP of the observation group were better than those of the control group,and the difference was statistically significant(t=2.224,2.228,3.292,4.131,4.394,3.831,3.064,2.511,2.128,P<0.05).After treatment,the indexes of BUN and Scr of the observation were(8.35±1.02)mmol/L,(110.49±10.67)μmol/L were lower than those of the control group(9.01±0.98)mmol/L,(116.13±10.15)μmol/L,the difference was statistically significant(t=3.060,2.511,P<0.05).Conclusion The therapeutic effect of sacubitril and valsartan is more prominent,which can effectively relieve cardiac insufficiency,improve renal function,and has high clinical application value.
