探讨早期康复护理对重症颅脑损伤患者的康复影响

Explore on the Effect of Early Rehabilitation Nursing on the Rehabilitation of Patients with Severe Head Injury

目的探讨早期康复护理对重症颅脑损伤患者的康复效果。方法随机选取2018年6月—2020年1月该院收治的60例重症颅脑损伤患者作为研究对象,前瞻性随机数表法将其分为两组,各30例。对照组行常规护理,研究组在此基础上增加早期康复护理,对比患者肌力情况、镇痛镇静效果评分、神经功能、生活自理能力。结果护理后,研究组上肢肌力、下肢肌力Ⅲ级及以上例数高于对照组(73.33%vs 16.67%,70.00%vs 413.33%),差异有统计学意义(χ^(2)=19.461、19.817,P<0.05);NRS、RASS镇静评分低于对照组[(2.31±1.02)分vs(4.32±1.82)分,(0.80±0.30)分vs(1.90±0.35)分],差异有统计学意义(t=5.277、13.070,P<0.05);GCS评分、Barthel指数评分高于对照组[(9.98±1.50)分vs(7.02±1.30)分,(58.28±12.05)分vs(39.58±11.02)分],差异有统计学意义(t=8.168、6.272,P<0.05)。结论重症颅脑损伤患者接受早期康复护理,可有效提高上下肢肌力与生活自理能力,改善神经缺损症状,该模式具有积极推广意义。

Objective To explore the rehabilitation effect of early rehabilitation nursing on patients with severe head injury.Methods Sixty patients with severe craniocerebral injury admitted to the hospital from June 2018 to January 2020 were randomy selected as the research objects.They were divided into two groups by prospective random number table method,30 cases in each group.The control group received routine nursing care.The research group added early rehabilitation care on this basis,and compared patients'muscle strength,analgesia and sedation effect scores,nerve function,and self-care ability.Results After nursing,the number of upper limb muscle strength and lower limb muscle strength grade III and above cases in the research group were higher than those in the control group(73.33%vs 16.67%,70.00%vs 413.33%),the difference was statistically significant(χ^(2)=19.461,19.817,P<0.05);NRS and RASS sedation scores were lower than those of the control group[(2.31±1.02)points vs(4.32±1.82)points,(0.80±0.30)points vs(1.90±0.35)points],the difference was statistically significant(t=5.277,13.070,P<0.05);GCS score and Barthel index score were higher than the control group[(9.98±1.50)points vs(7.02±1.30)points,(58.28±12.05)points vs(39.58±11.02)points],the difference was statistically significant(t=8.168,6.272,P<0.05).Conclusion Early rehabilitation care for patients with severe head injury can effectively improve their upper and lower limb muscle strength and self-care ability,and improve the symptoms of neurological deficits.This model has positive promotion significance.
