基于文献调研医疗机构制剂的使用情况及前景分析

Analysis on the Usage and Prospect of Hospital Preparation Based on Literature

目的:通过检索各医疗机构制剂文献报道的情况,分析医疗机构制剂的意义、面临的困境及机遇,探索医疗机构制剂的发展措施。方法:以"医院制剂""医疗机构制剂""院内制剂"等为关键词检索中国知网近十年报道的相关文献,整理、分析2010—2020年各医院报道的医疗机构制剂使用、销售、生产、研发情况。结果:纳入29篇文献,涉及中医医院12家、综合性医院13家和专科医院3家。大部分医院医疗机构制剂近十年的销售额均明显增加,主要以中药制剂为主,化药制剂数量相对较少,但中药制剂品种数量明显减少,新品种制剂较少。中药制剂中儿童专科医院中药制剂销售额较高。各医院专科均有医疗机构制剂,部分形成了特色专科,且使用量相对较高。结论:医疗机构制剂具有弥补市售药品不足、疗效明确、带动特色专科发展、促进医院科研及中药新药研发、满足突发状况等意义。目前医疗机构制剂生存发展的主要矛盾是投入与产出不成比例、老品种价格倒挂、医院不重视、新品种制剂申报审批数量少、转让难等问题。随着国家鼓励和扶持医疗机构使用中药制剂和新药审批相关政策出台,医疗机构制剂的发展迎来机遇与挑战,优化品种、突出特色、开展临床研究、加强新药转化、合理调整价格、扩大调剂使用等措施有利于医疗机构制剂的发展。

Objective:The significance,difficulties and opportunities of pharmaceutical preparations in medical institutions were analyzed by searching the literatures about pharmaceutical preparations in medical institutions,and the development measures of pharmaceutical preparations in medical institutions were explored.Methods:"Hospital preparations","medical institutions preparations"and"hospital preparations"were used as keywords to search relevant literatures reported by Cnki.net/in the past 10 years,and the use,sales,production and research and development of medical institutions preparations reported by various hospitals from 2010 to 2020 were sorted out and analyzed.Results:This study included 29 literatures,involving 12 traditional Chinese medicine hospitals,13 general hospitals and three specialized hospitals.In recent 10 years,the sales volume of pharmaceutical preparations in most hospitals and medical institutions increased obviously,mainly in traditional Chinese medicine preparations,and the quantity of pharmaceutical preparations was relatively small.However,the number of varieties of traditional Chinese medicine preparation decreased significantly,and the number of new varieties was less.In the Chinese medicine preparation,the sales volume of Chinese medicine preparation in children’s specialized hospital is higher.Each hospital specialized subject all has the medical establishment preparation,and part has formed the characteristic specialized subject.And the usage amount is relatively high.Conclusion:The preparation of medical institutions has the significance of making up for the shortage of drugs on the market,having clear curative effect,driving the development of specialty specialties,promoting hospital scientific research and the research and development of new traditional Chinese medicine,and satisfying the emergency situation.At present,the main contradictions in the survival and development of pharmaceutical preparations in medical institutions are the disproportionate input and output,the inverted price of old varieties,the lack of attention paid by hospitals,the lack of application and approval for new varieties,and the difficulty in transfer.With the introduction of relevant policies to encourage and support the medical institutions use of TCM and new drug approval related policies,the development of the medical establishment preparation has ushered in opportunities and challenges.To optimize the varieties,prominent characteristics,conducting clinical research,strengthening the new transformation,adjusting the price reasonable,and expanding regulate use measures is conducive to the development of preparation of medical institutions.
