经阴道超声造影检查对子宫内膜癌肌层浸润深度的诊断价值

Diagnostic significance of contrast-enhanced transvaginal ultrasound for depth of myometrial invasion in endometrial cancer

目的探讨经阴道超声造影检查对子宫内膜癌肌层浸润深度的诊断价值。方法选取2019年9月至2020年12月间西安国际医学中心医院收治的71例子宫内膜癌患者作为研究对象,所有患者予以经阴道超声造影检查以及术后病理检查,判断子宫内膜癌肌层浸润深度,对比两种检查方式的结果。结果术前经阴道超声造影检查可见,在71例子宫内膜癌患者中,检查结果为Ⅰa、Ⅰb、Ⅰc的患者分别为15例、20例、13例;术后病理检查可见,Ⅰa、Ⅰb、Ⅰc的患者分别为20例、25例、26例。两种检查方式检查结果的总符合率为67.6%,诊出率比较,Kappa值为0.457,两种检查方式存在中等一致性。结论经阴道超声造影检查可有效判断子宫内膜癌肌层浸润深度,准确率较高,具有一定的推广价值,但是仍然需要进一步开展深入检查加以证实。

Objective To explore the diagnostic value of contrast-enhanced transvaginal ultrasound for the depth of myometrial invasion in endometrial cancer. Methods A total of 71 patients with endometrial cancer admitted to Xi’an International Medical Center Hospital were selected from September 2019 to December 2020. The patients received transvaginal ultrasound and postoperative pathological examination for the depth of myometrial invasion in endometrial cancer. The results of the two inspection methods were compared. Results On preoperative contrast-enhanced transvaginal ultrasound examination,15,20 and 13 patients were diagnosed with stage Ⅰa,stage Ⅰb,and stage Ⅰc endometrial cancer,respectively. Postoperative pathological examination showed that 20,25 and 26 patients were diagnosed with stage Ⅰa,stage Ⅰb,and stage Ⅰc endometrial cancer,respectively. By comparing the two inspection methods,the overall coincidence rate was 67. 6%. The Kappa value was 0. 457. There was moderate consistency between the two inspection methods. Conclusion Contrast-enhanced transvaginal ultrasound can effectively determine the depth of myometrial invasion in endometrial cancer,with extremely high accuracy and certain promotion value.
