前庭型偏头痛发病机制的研究进展

Advances in the pathogenesis of vestibular migraine

前庭型偏头痛(VM)是常见的发作性眩晕疾病,临床主要表现为反复发作的眩晕,可伴头痛、恶心、呕吐、畏光、畏声、恐嗅等。人群患病率在眩晕疾病中仅次于良性阵发性位置性眩晕,然而其发病机制尚未完全明确。三叉神经血管系统(TVS)激活、神经递质释放异常、离子通道基因缺陷、中枢信号整合异常、皮质扩散抑制、遗传等方面可能与VM的发生、发展有关,但具体机制有待进一步研究。

Vestibular migraine is a common paroxysmal vertigo disease,and the main clinical manifestation is recurrent vertigo,which may be accompanied by headache,nausea and vomiting,photophobia and so on.The population prevalence of the disease is secondly only to benign paroxysmal positional vertigo in vertigo disorders.However,its pathogenesis has not been fully clarified.The activation of trigeminovascular system,abnormal neurotransmitter release,ion channel gene defects,vestibular and migraine pathways,cortical spreading depression,genetics and other aspects may be related to the occurrence and development of vestibular migraine,but the specific mechanism needs further studies.
