循环肿瘤细胞在非小细胞肺癌患者中的水平及其与肿瘤标志物的关系研究 被引量:3

摘要 目的探讨循环肿瘤细胞(CTCs)在非小细胞肺癌(NSCLC)患者中的水平及其与肿瘤标志物的关系,以为临床防治NSCLC提供一定的依据。方法回顾性选取2020年3月至2021年2月江门市中心医院收治NSCLC患者50例作为研究对象,男29例、女21例,<60岁23例、≥60岁27例。并整理与分析其临床资料,所有患者均进行CTCs检查,按照其水平将其分为阳性组(30例,≥4%)、阴性组(20例,<4%)。比较外周CTCs在不同性别、年龄、病理类型、分期的NSCLC患者中阳性率情况,及其与血清肿瘤标志物的关系。计量资料采用独立样本t检验,计数资料采用χ^(2)检验,用Spearman分析相关性。结果在NSCLC患者,阳性组、阴性组的性别、年龄、病理类型、TNM分期经比较差异均无统计学意义(均P>0.05),而与阳性组相比,阴性组Ⅲ期占比[45.00%(9/20)]高于阳性组[16.67%(5/30)],与阳性组相比,阴性Ⅳ期占比[30.00%(6/20)]低于阳性组[70.00%(21/30)]。在NSCLC患者,与阳性组相比,阴性组的血清糖类抗原(CA125)、癌胚抗原(CEA)、细胞角蛋白19片段(CYFRA21-1)水平均降低(均P<0.05)。外周CTCs水平与血清CEA、CA125、CYFRA21-1水平均呈正相关关系(r=0.445、0.436、0.423,均P<0.05)。结论在NSCLC患者中,外周CTCs计数水平与性别、年龄、病理类型、TNM分期无明显关系,而在临床Ⅲ期患者中外周CTCs计数水平较低,Ⅳ期患者外周CTCs计数水平较高,且外周CTCs水平与血清肿瘤标志物水平均呈正相关关系。
