缺血性脑小血管病患者血清β淀粉样蛋白1-42、脑源性神经营养因子、胰岛素样生长因子-1水平与血管性认知功能障碍的相关性分析 被引量:4

摘要 目的探讨缺血性脑小血管病患者血清β淀粉样蛋白1-42(Aβ1-42)、脑源性神经营养因子(BDNF)、胰岛素样生长因子-1(IGF-1)水平与血管性认知功能障碍(VCI)的相关性。方法2018年12月至2020年12月我院收治的86例缺血性脑小血管病患者,根据有无VCI分为VCI组和认知功能正常组。比较两组血清Aβ1-42、BDNF、IGF-1水平并分析三项指标与蒙特利尔认知评估量表(MoCA)评分的相关性。评估血清Aβ1-42、BDNF、IGF-1水平对缺血性脑小血管病患者VCI的诊断价值。结果VCI组血清Aβ1-42、BDNF、IGF-1水平低于认知功能正常组(P<0.05)。三项指标与MoCA评分均呈负相关(P<0.05)。血清Aβ1-42、BDNF、IGF-1及联合检测的AUC分别为0.795、0.727、0.734、0.911。结论缺血性脑小血管病患者血清Aβ1-42、BDNF、IGF-1水平参与到认知功能障碍的发生发展过程,对于诊断缺血性脑小血管病患者是否出现VCI具有重要意义。 Objective To investigate the correlation between the levels of serumβamyloid 1-42(Aβ1-42),brain-derived neurotrophic factor(BDNF)and insulin-like growth factor-1(IGF-1)and the vascular cognitive impairment(VCI)in patients with ischemic cerebral small vessel disease(CSVD).Methods Eighty-six patients with ischemic CSVD admitted to our hospital between December 2018 and December 2020 were divided into a VCI group and a normal cognitive function group according to the presence or absence of VCI.The levels of serum Aβ1-42,BDNF and IGF-1 were compared between the two groups.The correlation between the three indicators and the Montreal Cognitive Assessment Scale(MoCA)score was analyzed.The diagnostic value of serum Aβ1-42,BDNF and IGF-1 for VCI in patients with ischemic CSVD was evaluated.Results The levels of serum Aβ1-42,BDNF and IGF-1 in the VCI group were lower than those in the normal cognitive function group(P<0.05).The three indicators were negatively correlated with the MoCA score(P<0.05).The AUC values of serum Aβ1-42,BDNF,IGF-1 and combined detection were 0.795,0.727,0.734 and 0.911,respectively.Conclusion The levels of serum Aβ1-42,BDNF and IGF-1 of patients with ischemic CSVD are involved in the occurrence and development of cognitive impairment.The indicators are of great significance for the diagnosis of VCI in patients with CSVD.
