miR-133b、miR-33对冠状动脉疾病的早期预测价值分析 被引量:3

摘要 目的探讨微小核糖核酸(miR)-133b、miR-33对冠状动脉疾病(CAD)的早期预测价值。方法纳入2017年8月至2021年5月收治的CAD患者124例为CAD组,选取同期体检的健康志愿者87例为对照组,检测两组血清miR-133b、miR-33相对表达水平,同时检测血清心肌肌钙蛋白T(cTnT)、肌钙蛋白I(cTnI)、白介素(IL)-6、IL-17、C反应蛋白(CRP)水平,并采用Gensini积分评估冠脉病变程度,分析miR-133b、miR-33与心肌损害指标、炎症指标、Gensini积分的相关性。根据Gensini积分将患者分成轻度组、中度组、重度组,比较三组miR-133b、miR-33表达水平,绘制受试者操作特征(ROC)曲线,分析二者诊断CAD及评估其严重程度的曲线下面积(AUC)。结果CAD组血清miR-133b、miR-33表达及cTnT、cTnI、IL-6、IL-17、CRP高于对照组(P<0.05)。血清miR-133b、miR-33表达与cTnT、cTnI、IL-6、IL-17、CRP及Gensini积分呈正相关(P<0.05)。轻度、中度组血清miR-133b、miR-33表达低于重度组,且轻度组低于中度组(P<0.05)。miR-133b、miR-33单独及联合诊断CAD的AUC分别为0.787、0.771、0.825,二者单独及联合评估CAD严重度的AUC分别为0.761、0.788、0.821。结论CAD患者的血清miR-133b、miR-33呈过表达,二者对CAD诊断及严重度评估有一定价值,有望成为早期CAD的预测指标。 Objective To investigate the value of microRNA(miR)-133 b and miR-33 in the early prediction of coronary artery disease(CAD).Methods 124 patients with CAD treated in our hospital from August 2017 to May 2021 were included in the CAD group,and 87 healthy volunteers who underwent physical examination in the same period were selected as the control group.The relative expression levels of miR-133 b and miR-33 in serum and the levels of cardiac troponin T(cTnT),cardiac troponinⅠ(cTnⅠ),interleukin(IL)-6,IL-17 and C-reactive protein(CRP)in serum were detected.The degree of coronary artery disease was evaluated by Gensini score.The patients were divided into mild group,moderate group,and severe group according to Gensini score.The expression levels of miR-133 b and miR-33 in the three groups were compared.Receiver operating characteristic(ROC)curve was drawn to analyze the area under the curve(AUC)for diagnosing CAD and assessing its severity.Results The expression of serum miR-133 b,miR-33,cTnT,CTnⅠ,IL-6,IL-17 and CRP in CAD group were higher than those in control group(P<0.05).The expression of miR-133 b and miR-33 in serum were positively correlated with cTnT,CTnⅠ,IL-6,IL-17,CRP and Gensini score(P<0.05).The expressions of serum miR-133 b and miR-33 in mild and moderate groups were lower than those in severe group,and those in mild group were lower than those in moderate group(P<0.05).The AUCs of miR-133 b and miR-33 alone and jointly diagnosed CAD were 0.787,0.771 and 0.825,respectively,and the AUCs of the two alone and jointly evaluated CAD severity were 0.761,0.788 and 0.821,respectively.Conclusion The serum miR-133 b and miR-33 in patients with CAD are overexpressed.They have certain value in the diagnosis and severity evaluation of CAD,and are expected to become predictors of early CAD.
