超声骨刀在腰椎管狭窄症全椎板切除减压术中的应用研究

Application of Ultrasonic Osteotome in Total Laminectomy and Decompression for Lumbar Spinal Stenosis

目的:探讨超声骨刀在腰椎管狭窄症全椎板切除减压术中的应用效果。方法:选择2019年1月~2020年12月本院收治的102例腰椎管狭窄症患者,以随机数字表法划分为两组各51例。两组均行全椎板切除减压术,对照组术中行传统椎板咬骨钳咬除操作,观察组采用超声骨刀实施骨切割操作。比较两组手术时间、术中失血量、引流量、住院时间,术前与术后日本骨科学会(JOA)评分与视觉模拟评分(VAS),以及并发症情况。结果:观察组术中失血量、引流量、住院时间较对照组更低(P<0.01)。术后两组JOA、VAS评分无明显差异(P>0.05)。观察组并发症发生率1.96%较对照组15.69%更低(P<0.05)。结论:超声骨刀在腰椎管狭窄症全椎板切除减压术中具有显著的应用效果。

Objective:To investigate the application effect of ultrasonic osteotome in total laminectomy and decompression for lumbar spinal stenosis.Methods:From January 2019 to December 2020,102 patients with lumbar spinal stenosis were randomly divided into two groups,51 cases in each group.All patients underwent laminectomy and decompression.The control group was treated with laminectomy and the study group was treated with ultrasonic osteotome.The operation time,blood loss,flow rate,hospitalization time,JOA score,visual simulation score(VAS)and complications were compared between the two groups.Results:The intraoperative blood loss,drainage volume and hospitalization time of the study group were lower than those of the control group(P<0.01).There was no significant difference in JOA and VAS score between the two groups after treatment(P>0.05).The incidence of complications in the study group was 1.96%,which was lower than 15.69%in the control group(P<0.05).Conclusion:Ultrasonic osteotome has significant effect in total laminectomy and decompression for lumbar spinal stenosis.
