联合监测IL-8、TNF-α及血气在新生儿胎粪吸入综合征中的临床意义探讨

目的:探讨胎粪吸入综合征(MAS)新生儿血中白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)及血气分析的临床价值,为疾病诊治提供参考。方法:选取2019年7月~2020年5月在本院治疗的MAS新生儿62例为观察组,包括轻症34例、重症28例,同时选择非感染新生儿40例为对照组,比较IL-8、TNF-α水平,以及脐动脉血气。结果:出生1d,MAS组新生儿血清IL-8、TNF-α检测值高于对照组(P<0.05),重症MAS患儿血清IL-8、TNF-α水平明显高于轻症患儿(P<0.05);经治疗,IL-8、TNF-α呈逐渐下降趋势(P<0.05)。MAS组新生儿脐动脉血的动脉血二氧化碳分压(PaCO_(2))高于对照组,pH、碱剩余(BE)、碳酸氢根(HCO_(3)^(-))低于对照组,差异有统计学意义(P<0.05)。结论:新生儿胎粪吸入综合征血清IL-8、TNF-α水平和血气分析发生明显改变,联合检测对于病情评估及疗效评价有重要意义。

Objective:To investigate the clinical value of serum interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)and blood gas analysis in neonates with meconium aspiration syndrome(MAS)and provide reference for disease diagnosis and treatment.Methods:62 neonates with MAS treated in our hospital from July 2019 to May 2020 were selected as the observation group,including 34 cases of mild disease and 28 cases of severe disease,and 40 neonates without infection during the same period were selected as the control group.Comparing the levels of IL-8,TNF-αand umbilical artery blood gas.Results:One day after birth,the serum levels of IL-8 and TNF-αin the MAS group were higher than those in the control group(P<0.05),The serum IL-8 and TNF-αlevels in critically ill children were significantly higher than those in mild children(P<0.05);After treatment,IL-8 and TNF-αshowed a gradually decreasing trend(P<0.05).The arterial partial pressure of carbon dioxide(PaCO_(2))of neonatal umbilical artery in MAS group was higher than that in control group,and the pH,buffuer excess(BE)and bicarbonate(HCO_(3)^(-))were lower than those in control group(P<0.05).Conclusion:The serum levels of IL-8,TNF-αand blood gas analysis in neonatal meconium aspiration syndrome are significantly changed.The combined detection is of great significance for disease assessment and curative effect evaluation.
