感兴趣区选取数量对脑胶质瘤IVIM参数测量的影响

摘要 目的探讨感兴趣区选取数量对脑胶质瘤IVIM参数测量的影响。方法回顾性分析2019年5月-2020年8月我院收治的56例胶质瘤患者治疗前MRI平扫、增强和IVIM-DWI图像,通过后处理软件测量肿瘤实质区、瘤周水肿区及对侧正常白质的表观扩散系数(ADC)、真扩散系数(D)、伪扩散系数(D*)、灌注分数(f),采用小圆形ROI勾画法(30~45 mm^(2))对以上三种不同区域分别选取3个(3点法)、5个(5点法)、10个(10点法)感兴趣区,比较不同数量ROI所得参数值的差异。结果三种ROI方法不同区域ADC、D*、D、f比较,差异无统计学意义(P>0.05),但三种方法测得肿瘤实质区及瘤周水肿区的IVIM参数可重复性高(ICC>0.8),测量对侧正常白质IVIM参数的可重复性较好(ICC>0.6);肿瘤实质区、瘤周水肿区、对侧正常白质的ADC、D*、D值比较,差异有统计学意义(P<0.05),且肿瘤实质区、瘤周水肿区的ADC、D值高于对侧正常白质,D*值低于对侧;三区域间f值比较,差异无统计学意义(P>0.05)。测量肿瘤实质区、瘤周水肿区、对侧正常白质的ADC、D值时,5点法、10点法(AUC=0.7)诊断效能优于3点法(AUC<0.7);测量D*、f时,三种ROI方法AUC均<0.7。结论选取不同数量感兴趣区获得的脑胶质瘤IVIM参数具有较高的可重复性,但在比较不同区域IVIM参数差异时,5点法及10点法测量ADC、D的性能优于3点法,三种方法均不适合测量D*及f。 Objective To explore the influence of the number of selected ROIs on the measurement of IVIM parameters of glioma.Methods The plain,enhanced and IVIM-DWI images of 56 patients with glioma before treatment in our hospital from May 2019 to August 2020 were retrospectively analyzed.The apparent diffusion coefficient(ADC),true diffusion coefficient(D),pseudo diffusion coefficient(D*),and perfusion fraction(f)of tumor parenchyma,peritumoral edema and contralateral normal white matter were measured by post-processing software.Three(3-point method),five(5-point method),and ten(10-point method)regions of interest were selected for the above three different regions by using the small circle ROI mapping method(30-45 mm^(2)),The parameter values of different ROIs were compared.Results There was no significant difference in ADC,D*,D and f among the three ROI methods in different regions(P>0.05),however,the repeatability of IVIM parameters measured by the three ROI methods in tumor parenchyma and peritumoral edema was high(ICC>0.8),and the repeatability of IVIM parameters measured in contralateral normal white matter was good(ICC>0.6).The ADC,D*and D values of tumor parenchyma,peritumoral edema and contralateral normal white matter were compared,and the differences were statistically significant(P<0.05).When ADC and D values of tumor parenchyma,peritumoral edema and contralateral normal white matter were measured,the diagnostic efficacy of 5-point method and 10-point method(AUC=0.7)was better than that of 3-point method(AUC<0.7).When D*and f were measured,the AUC of three ROI methods were<0.7.Conclusion The IVIM parameters of glioma obtainedby selecting different numbers of regions of interest have high reproducibility.However,when comparing the differences of IVIM parameters in different regions,the performance of 5-point method and 10-point method in measuring ADC and D is better than that of 3-point method,and the three methods are not suitable for measuring D*and f.
