摘要
目的探讨血清高敏C-反应蛋白/白蛋白比值(CAR)与假定血管源性脑白质高信号(WMH)严重程度的相关性。方法选择2020年2月-2021年2月安徽医科大学附属巢湖医院神经内科住院及门诊确诊的WMH患者119例,依据Fazekas评分将WMH患者划分为WMH1级组、WMH2级组和WMH3级组。收集各组患者的一般资料、总胆固醇(TC)、同型半胱氨酸(HCY)、高敏C反应蛋白(hs-CRP)、白蛋白(ALB),计算CAR并进行比较,分析CAR水平与WMH严重程度的相关性。结果三组年龄、高血压、既往卒中史、HCY、hs-CRP、ALB、CAR两两比较,差异有统计学意义(P<0.05);相关性分析显示,CAR与WMH严重程度呈正相关(r=0.574);单因素Logistic回归分析显示,年龄、高血压、既往卒中史、hs-CRP、CAR是WMH严重程度增加的危险因素;多因素Logistic回归分析显示,年龄、高血压、CAR是WMH严重程度增加的独立危险因素。结论CAR与WMH程度呈正相关,对WMH病情的评估具有辅助作用。
Objective To investigate the correlation between serum high-sensitivity C-reactive protein/albumin ratio(CAR)and the severity of assumed vascular-derived white matter hypertensity(WMH).Methods A total of 119 patients with WMH diagnosed in the Department of Neurology,Chaohu Hospital Affiliated to Anhui Medical University from February 2020 to February 2021 were selected.According to the Fazekas score,the patients with WMH were divided into WMH1 group,WMH2 group and WMH3 group.The general data,total cholesterol(TC),homocysteine(HCY),high-sensitivity C-reactive protein(hs-CRP)and albumin(ALB)of each group were collected,and CAR was calculated and compared to analyze the correlation between CAR level and WMH severity.Results There were significant differences in age,hypertension,previous stroke history,HCY,hs-CRP,ALB and CAR among the three groups(P<0.05).Correlation analysis showed that CAR was positively correlated with WMH severity(r=0.574).Univariate Logistic regression analysis showed that age,hypertension,previous stroke history,hs-CRP and CAR were risk factors for increased severity of WMH.Multivariate Logistic regression analysis showed that age,hypertension and CAR were independent risk factors for increased severity of WMH.Conclusion CAR is positively related to the degree of WMH,and can be used as a disease assessment of WMH.
