DKI在直肠癌病理分级和分期中的应用价值 被引量:1

The Application Value of Diffusion Kurtosis Imaging in Pathological Grading and Staging of Rectal Adenocarcinoma

摘要 目的:评估弥散峰度成像(DKI)参数K值、D值和表观弥散系数(ADC)值在直肠癌诊断、分化程度评估和分期中的价值。方法:共纳入38例直肠腺癌患者进行前瞻性研究,术前行直肠高分辨率磁共振和DKI检查,勾画感兴趣区,获得K值、D值和ADC值,然后进行统计分析。结果:直肠癌肿瘤实质区与正常肠壁K值、D值及ADC值差异均具有统计学意义(P<0.05)。DKI定量D值及ADC值在不同分化程度组中差异也均具有统计学意义(P<0.05),但K值差异无统计学意义(P>0.05)。DKI定量K值和D值及ADC值在T2、T3和T4中差异具有统计学意义(P<0.05)。结论:DKI定量参数对于临床诊断直肠癌、评估和预测侵袭性方面具有重要的临床价值。 Purpose: To evaluate the value of diffusion kurtosis imaging(DKI) parameter K value, D value and ADC value in diagnosis, pathological grading and staging of rectal carcinoma.Methods: A total of 38 patients with rectal carcinoma were selected in this prospective research. All patients underwent high-resolution magnetic resonance imaging and DKI examination before operation. K values, D values and apparent diffusion coefficient(ADC) values were obtained by drawing the region of interest(ROI) and statistical analysis was performed.Results:The K value, D value and ADC value of tumor parenchyma area of the rectal cancer showed statistical difference with the normal intestinal wall(P<0.05). DKI quantitative D value and ADC value in different differentiation groups were with statistical difference(P<0.05), but K value was with no statistical significant(P>0.05). DKI quantitative K value, D value and ADC value were with statistical difference in T2, T3 and T4(P<0.05).Conclusion: Quantitative parameters of DKI have important clinical value for clinical diagnosis of rectal cancer, assessment and prediction of invasiveness.
