泛免疫炎症值及全身免疫炎性指数对胃癌预后的评估价值

Prognostic Value of Pan-Immune-Inflammation Value and Systemic Immune-Inflammation Index in Gastric Cancer

目的探讨术前炎性反应标志物、泛免疫炎症值(PIV)及全身免疫炎性指数(SII)对胃癌患者预后的评估价值。方法回顾分析2016年3月~2019年12月在徐州医科大学附属医院确诊为胃癌的396例患者的临床病理资料,通过5年时依性ROC曲线确定PIV及SII的最佳截断值,将两者分为高PIV、低PIV组及高SII、低SII组,分别比较组间临床病理特征关系;通过Kaplan-Meier法和Log-Rank检验进行单因素分析并绘制生存曲线,分析PIV和SII值在评估胃癌患者预后中的价值。结果PIV及SII与胃癌患者的肿瘤浸润程度、淋巴结转移、TNM分期、神经或脉管侵犯、肿瘤直径均有关,差异有统计学意义(P<0.05),高PIV组的患者5年生存率低于低PIV组(57.8%vs 82.1%),高SII组的5年生存率低于低SII组(60.0%vs 81.5%),差异均有统计学意义(P<0.05)。结论术前高PIV及高SII均有助于预测胃癌患者的预后,且PIV对胃癌预后的预测价值好于SII。

Objective To evaluate the prognostic value of preoperative inflammatory markers,PIV and SII in patients with gastric cancer.Methods The clinicopathological data of 396 patients diagnosed with gastric cancer in the Affiliated Hospital of Xuzhou Medical University from March 2016 to December 2019 were analyzed retrospectively.The best cut-off values of PIV and SII were determined according to the time-dependent ROC curve at 5 years.The PIV and SII were divided into the elevated group and the decreased group.The clinicopathological characteristics of the two groups were compared.Univariate analysis was performed by Kaplan-Meier method and Log-Rank test,and the survival curve was drawed to analyze the value of PIV and SII in evaluating the prognosis of gastric cancer patients.Results PIV and SII were associated with the degree of tumor invasion,lymph node metastasis,TNM stage,nerve or vascular invasion,tumor diameter,in patients with gastric cancer(P<0.05).The 5-year survival rate of patients with high PIV value was lower than that of patients with low PIV value(57.8%vs 82.1%).The 5-year survival rate of patients with high SII value was lower than that of patients with low SII value(60.0%vs 81.5%).And the difference was ststistically significant(P<0.05).Conclusion Preoperative high PIV and high SII is helpful to predict the prognosis of gastric cancer patients,and the predictive value of PIV is better than SII.
