经尿道钬激光前列腺剜除术对良性前列腺增生患者的治疗效果

Effects of HoLEP on Urination Improvement and Serum PSA Level in Patients with BPH

目的:探讨经尿道钬激光前列腺剜除术(HoLEP)治疗良性前列腺增生(BPH)患者的效果。方法:回顾性分析101例BPH患者临床资料,根据治疗方法不同分组,行经尿道前列腺电切术(TURP)的50例患者为对照组,行HoLEP术的51例患者为观察组。比较2组患者手术相关指标、国际前列腺症状评分(IPSS)、最大尿流率(Qmax)、残余尿量(RUV)、前列腺特异性抗原(PSA)及术后并发症情况。结果:观察组手术时间长于对照组[(95.66±14.48)vs(69.05±10.22)min],术中出血量[(32.51±5.33)vs(89.55±12.45)mL]、膀胱冲洗时间[(1.44±0.29)vs(1.86±0.30)d]、尿管留置时间[(4.60±0.46)vs(6.60±0.46)d]、术后住院时间[(5.86±1.60)vs(7.30±1.63)d]均低于对照组(P<0.05);观察组治疗后IPSS评分[(8.44±1.22)vs(12.49±1.83)分]、Qmax[(19.45±4.21)vs(17.11±4.12)mL·s^(-1)]、RUV[(16.33±3.84)vs(18.26±4.22)mL]优于对照组(P<0.05);观察组PSA[(1.05±0.29)ng·mL^(-1)]低于对照组[(1.49±0.34)ng·mL^(-1)];观察组术后并发症发生率5.88%低于对照组的22.00%(P<0.05)。结论:与TURP术比较,HoLEP用于治疗BPH术后并发症发生率低,疗效好。

Objective:To study the therapeutic effect of transurethral holmium laser enucleation of the prostate(HoLEP)in patients with benign prostatic hyperplasia(BPH).Methods:the clinical data of 101 patients with BPH were reviewed.According to different treatment methods,50 patients undergoing TURP were the control group and 51 patients undergoing HoLEP were the observation group.The operation related indexes,International Prostate Symptom Score(IPSS),maximum urinary flow rate(Qmax),residual urine volume(RUV),laboratory indexes(PSA)and postoperative complications were compared between the two groups.Results:the operation time[(95.66±14.48)vs(69.05±10.22)min]in the observation group was higher than that in the control group,and the intraoperative bleeding[(32.51±5.33)vs(89.55±12.45)mL],bladder flushing time[(1.44±0.29)vs(1.86±0.30)d],urinary catheter retention time[(4.60±0.46)vs(6.60±0.46)d]and postoperative hospital stay[(5.86±1.60)vs(7.30±1.63)d]in the observation group were lower than those in the control group(P<0.05);IPSS score[(8.44±1.22)vs(12.49±1.83)points],Qmax[(19.45±4.21)vs(17.11±4.12)mL·s^(-1)]and RUV[(16.33±3.84)vs(18.26±4.22)mL]in the observation group were better than those in the control group(P<0.05);PSA in the observation group[(1.05±0.29)ng·mL^(-1)]was lower than that in the control group[(1.49±0.34)ng·mL^(-1)];The incidence of postoperative complications in the observation group was 5.88%(3/51)lower than 22.00%(11/50)in the control group(P<0.05).Conclusion:compared with TURP,HoLEP has low incidence of postoperative complications and significant curative effect in the treatment of BPH,which is worthy of clinical application.
