对比声触诊弹性成像与可视化瞬时弹性成像检测肝脏弹性

Comparison on sound touch elastography and visual transient elastography for measuring liver elasticity

目的对比观察声触诊弹性成像(STE)与可视化瞬时弹性成像(ViTE)检测肝脏弹性的价值。方法收集478名接受腹部超声检查的受检者,根据检查结果将其分为肝纤维化组(n=147)、肝硬化组(n=162)及健康组(n=169);采用STE和ViTE测量各组肝脏弹性值,观察2种方法检测的一致性、检测成功率、稳定性及其测值的差异。结果STE和ViTE检测成功率差异无统计学意义(P>0.05),其对肝纤维化组及健康组的检测一致性均较好,对肝硬化组的一致性较差。STE及ViTE针对肝纤维化组和健康组的测量结果的稳定性均较好;对肝硬化组,STE稳定性较好,而ViTE稳定性较差。STE与ViTE所测肝纤维化组及健康组肝脏弹性值差异均无统计学意义(P均>0.05),而STE测得肝硬化组肝脏弹性值显著低于ViTE(P<0.05)。STE及ViTE对男性受试者肝脏弹性值的测值均大于女性(P均<0.05),尤以25~50岁受检者为著(P均<0.05)。结论STE与ViTE检测肝脏弹性值的成功率均较高,且对肝纤维化患者和健康人群的一致性和稳定性均较好,而用于检测肝硬化患者的一致性较差;对肝硬化患者,STE检测稳定性较好,而ViTE较差。

Objective To compare the value of sound touch elastography(STE)and visual transient elastography(ViTE)for detecting measuring liver elasticity.Methods Totally 478 subjects who underwent abdominal ultrasonography were enrolled and divided into liver fibrosis group(n=147),liver cirrhosis group(n=162)and healthy group(n=169)according to ultrasonic findings.The liver elasticity was measured with STE and ViTE.The consistency,measuring success rate,measuring stability and measured values of these 2 methods were observed and compared.Results There was no significant difference of measuring success rate of STE and ViTE(P>0.05).The consistency of liver elasticity measured with STE and ViTE in liver fibrosis group and healthy group were both good,but in liver cirrhosis group was poor.The stability of liver elasticity measured with STE and ViTE in liver fibrosis group and healthy group were both good,while in liver cirrhosis group,the stability of STE was good,but the stability of ViTE was poor.There was no significant difference of liver elasticity measured with STE and ViTE in liver fibrosis group nor in healthy group(both P>0.05),but liver elasticity measured with STE was significantly lower than that measured with ViTE in liver cirrhosis group(P<0.05).Both STE and ViTE showed higher liver elasticity in males than in females(both P<0.05),especially for people aged 25—50 years(both P<0.05).Conclusion The success rates of STE and ViTE for measuring liver elasticity were both high,and the consistency and stability in liver fibrosis patients and healthy subjects were both good,but the consistency in liver cirrhosis patients was poor.For liver cirrhosis patients,the stability of liver elasticity measured with STE was good,while with ViTE was poor.
