经阴道分娩产妇第三产程预防性应用麦角新碱联合缩宫素的临床效果观察

Clinical study on prophylactic application of ergometrine combined with oxytocin during the third stage of labor in women with vaginal delivery

目的:探讨预防性应用麦角新碱联合缩宫素对经阴道分娩产妇第三产程的临床效果。方法:选取我院2018-01~2020-01收治的经阴道分娩产妇122例作为研究对象,使用随机对照法将其分为研究组(n=61例)与对照组(n=61例),对照组产妇给予缩宫素预防,研究组产妇联合麦角新碱与缩宫素共同预防。记录并比较两组产妇产程中、产后2h及产后24h出血量;比较两组产妇产前、产后红细胞(RBC)、血红蛋白(Hb)及产后血细胞比容(HCT)变化情况;比较两组产妇产后恶心呕吐、头晕、心动过速等不良反应发生情况。结果:研究组产妇产程中、产后2h、产后24h出血量均低于对照组(P<0.05);产前两组产妇RBC、Hb、HCT无显著差异,产后研究组产妇RBC、Hb、HCT均高于对照组(P<0.05);研究组产妇不良反应总发生率9.84%,低于对照组29.4%(P<0.05)。结论:联合麦角新碱与缩宫素预防性用于经阴道分娩产妇第三产程,可有效减少产妇出血量,预防产后出血发生,安全性较高。

Objective:To explore the clinical efficacy of prophylactic application of ergometrine combined with oxytocin in third stage of vaginal delivery.Methods:A total of 122 cases of women with vaginal delivery admitted to Luoyang Oriental Hospital from January 2018 to January 2020 were selected as the study subjects and divided into study group and control group according to the randomized control method,with 61 cases in each group.Control group of maternal use of oxytocin prevention,study group of maternal combined ergoxin and oxytocin prevention.Blood loss during labor,2 hours postpartum and 24 hours postpartum were recorded and compared between the two groups.The changes of erythrocyte(RBC),hemoglobin(Hb)and hematocrit(HCT)were compared between the two groups before and after delivery.Postpartum nausea,vomiting,dizziness,tachycardia and other adverse reactions were compared between the two groups.Results:The amount of maternal bleeding in the study group was lower than that in the control group during labor,2 hours after delivery and 24 hours after delivery.There were no significant differences in RBC,Hb and HCT between the two groups before delivery,but the RBC,Hb and HCT in the postpartum study group were higher than those in the control group.The incidence of adverse maternal reactions in the study group was 9.84%lower than 29.4%in the control group.The differences were statistically significant(P<0.05).Conclusion:Prophylactic application of ergometrine combined with oxytocin during the third stage of labor can effectively reduce maternal bleeding and prevent postpartum hemorrhage,with high safety.
