喉癌术后患者气道湿化管固定方法的改良及应用

Improvement and application of airway humidification tube fixation in postoperative patients with laryngeal cancer

目的:探讨喉癌术后患者标准化的湿化管固定方法,为喉癌术后患者标准化护理管理准则提供参考。方法:选取2018-06~2019-01在河南省某三级甲等医院咽喉头颈外科住院的91例喉癌术后气管切开患者作为研究对象。按入院时间分为3组,“一”形固定组32例,蝶形固定组30例,弹力线固定组29例,“一”形固定组按照常规固定方法固定,蝶形固定组和弹力线固定组采用标准化固定方法固定,比较3组患者湿化管全天再固定次数、全天再固定总时间、全天再固定经济支出及湿化效果的差异。结果:3组间全天再固定次数差异显著,其中,弹力线固定组固定次数最少,“一”形固定组固定次数最多。且3组间全天再固定总时间、全天再固定经济支出差异具有统计学意义(P<0.05)。弹力线组湿化满意可达96.6%,显著高于蝶形固定组73.3%和“一”形固定组68.8%(P<0.05)。结论:标化后的湿化管固定方法在保证湿化效果的前提下,能显著提高护士工作效率,降低医疗成本。

Objective:To explore the improvement and application of airway humidification tube fixation in patients with postoperative laryngeal cancer,and to provide reference for the formulation of standardized nursing management criteria for patients with postoperative cancer.Methods:From June 2018 to January 2019,91 patients with tracheotomy after laryngeal cancer surgery in the Department of Laryngology,Head and Neck Surgery,The First Affiliated Hospital of Zhengzhou University were selected as the research object.According to the time of admission and different fixation methods,the patients were divided into three groups:uniform fixation group(n=32),butterfly fixation group(n=30)and elastic wire fixation group(n=29).The first fixed group was fixed by conventional fixation method,while the butterfly fixed group and elastic wire fixed group were fixed by standardized fixation method.The Times,total time,economic expenditure and humidification effect of humidification tube re-fixation were compared among the three groups.Results:There was statistically significant difference in the number of all-day re-fixation among the three groups(P<0.05).The elastic wire fixation group had the least number of fixation,while the one shape fixation group had the most.The total time of all-day re-fixation and economic expenditure of all-day re-fixation were significantly different among the three groups(P<0.05).The wetting satisfaction of the elastic wire group was 96.6%,that of the butterfly fixation group was 73.3%,and that of the one shape fixation group was 68.8%.The wetting satisfaction of the elastic wire group was the highest,and the difference between the three groups was statistically significant(P<0.05).Conclusion:The standardized operation of humidifying tube fixation method can improve the work efficiency of nurses and reduce the medical cost on the premise of ensuring the humidifying effect.
